Lancet Neurol:成年后血压与老年脑结构变化

从成年早期到中年,高血压和血压的增加与老年WMHV的增加和脑体积的缩小有关

中年人群高血压导致老年认知功能障碍风险增加。近日研究人员考察了成年期间血压或血压变化与晚期脑结构、病理学和认知的影响。

参与者来自Insight 46研究,接受MRI、Florbetapir淀粉样蛋白PET成像和认知评估,所有参与者均无痴呆。在36岁、43岁、53岁、60-64岁和69岁时收集了血压测量数据。研究的主要观察指标为MRI量化白质高强度体积(WMHV),淀粉样蛋白表达阳性或阴性,全脑和海马体积以及综合认知评分-临床前阿尔茨海默病认知复合物(PACC)。

465名参与者,其中男性238名(51%),平均年龄70.7岁。53岁时收缩压(SBP)和舒张压(DBP)升高,43-53岁时收缩压和舒张压(DBP)升高与69-71岁时的WMHV呈正相关(SBP每增加10 mm Hg,平均WMHV增加7%;DBP每增加10 mm Hg,平均WMHV增大15%)。43岁时DBP升高与69-71岁时全脑体积变小有关(每 10 mm Hg减少6.9 mL),年龄在36-43岁之间的DBP增加与脑体积减小显著相关(每 1 SD变化减少6.5 mL)。年龄在36至43岁之间SBP的增加与69-71岁时海马体积较小有关(每 1 SD 变化减少0.03 mL)。绝对血压和血压变化都不能预测69-71岁时淀粉样蛋白β的状态或PACC评分。

研究发现,从成年早期到中年,高血压和血压的增加与老年WMHV的增加和脑体积的缩小有关。
