研究人员利用监测、流行病学、最终结果登记和国家癌症数据库来确定从2007年到2014年被诊断为膀胱癌的且65岁以下的个体,并评估了保险状态与随后结果的关系。研究发现,与那些具有私人保险的相比,没有保险或者只有医疗保险的个体诊断为肌肉浸润性膀胱癌的比例高出1倍(没有保险个体OR, 1.90; 95%CI, 1.70-2.12; 医疗保险个体OR, 2.03; 95% CI, 1.87-2.20),并且他们由于膀胱癌死亡的可能性更高(没有保险个体AHR,1.49;95% CI, 1.31-1.71; 只有医疗保险个体AHRs, 1.61; 95% CI, 1.46-1.79)。另外,与具有私人保险的个体相比,治疗推迟90天的情况在没有保险(OR, 1.36; 95% CI, 1.12-1.65)或者只有医疗保险的个体(OR, 1.22; 95% CI, 1.03-1.44)中可能性更高。没有保险的个体在大医院进行治疗的可能性低,而具有医疗保险的个体则接受NAC治疗的比例更低。

最后,研究人员指出,与那些具有私人保险的个体相比,没有保险和只有医疗保险的个体的预后和护理均较差。因此,扩展高质量的医疗保险来覆盖普通大众也许能够减少该疾病的负担。
