本研究对二十二例前列腺癌患者进行术前3.0T磁共振成像。通过使用回波时间(47, 75, 100毫秒)和b值分别为0, 750, 1500秒/毫米×2的所有组合,获得混合多维MR成像轴位图像,从而产生与每个体素相关联的3×3阵列数据。通过将混合数据拟合到三区信号模型,计算每个分区的表观扩散系数(ADC)和T2值,计算组织成分间质、上皮和管腔的体积。测量前列腺切除术确诊恶性肿瘤的感兴趣区(n=28)和正常组织(n=71)的体积分数和常规ADC和T2。用受试者工作特征曲线(ROC)分析来评估前列腺癌和良性组织中的各种参数。

结果为与正常组织相比,混合多维磁共振成像显示前列腺癌组织上皮细胞体积分数明显增加(分别为23.2%±7.1 比48.8%±9.2)、管腔体积减小(26.4%±14.1 比14.0%±5.2)、基质体积减小(50.5% ±15.7比37.2%±9.1)。组织成分的体积分数显示相比较于T2值(ρ= -0.292,P =0 .132)和ADC值(ρ= -0.315,P =0 .102),Spearma与Gleason评分相关系数更高(上皮:ρ= 0.652,P = 0.0001;基质:ρ= -0.439,P = 0.020;管腔:ρ= -0.390,P =0.040)。ROC曲线下面积显示区分前列腺癌与正常前列腺时,差异最高的是上皮部分体积(0.991),其次是管腔部分体积(0.800)和基质(0.789)。

结论表明当癌症存在时,前列腺管腔,基质和上皮细胞的体积分数变化显着。这些参数可以通过使用混合多维磁共振成像非侵入性地测量,且有可能提高前列腺癌的诊断并确定其侵袭性。
