研究发现VAI水平是NAFLD的独立危险因素,VAI水平与NAFLD风险之间存在剂量-反应关系。

研究背景:虽然有证据表明内脏脂肪组织与非酒精性脂肪肝(NAFLD)有关,但内脏脂肪指数(VAI)能否预测NAFLD的发生尚不清楚。本研究旨在确定VAI是否是NAFLD的独立危险因素,是否存在剂量-反应关系。

研究方法:该研究在2012年建立的健康检查队列中探讨了VAI水平与NAFLD的关系。该研究纳入了4809例具有基线和4次随访检查结果的受试者。非酒精性脂肪肝的诊断是通过超声显示肝回声增强和问卷调查排除过量饮酒而确定的。研究将受试者按照基线VAI水平分为4组,采用Cox风险回归模型,通过调整潜在的混杂因素,在基线时通过四分位数估计NAFLD风险。采用Kaplan-Meier生存分析比较个体NAFLD发生的风险。

研究结果:该队列中NAFLD的4年累积发病率为13.9%。相比第一四分位数,NAFLD的第二、第三和第四四分位数的危害比(95%置信区间,95%CI)分别为1.42(95%CI: 1.24-1.64)、1.73(95%CI: 1.51-1.99)和2.13(95%CI: 1.86-2.45)。Kaplan-Meier生存分析表明,VAI水平越高,NAFLD的发生率越高,且与剂量相关。

研究结论:研究发现VAI水平是NAFLD的独立危险因素,VAI水平与NAFLD风险之间存在剂量-反应关系。
