研究认为,胃酸抑制增加多药耐药微生物定植风险。鉴于全球抗微生物耐药性威胁,减少不必要抑酸剂使用是十分迫切

抑制酸分泌对肠道微生物群有破坏作用,但酸抑制对多药耐药微生物(MDROs)定植的风险尚不清楚。近日研究人员开展荟萃研究,考察了酸抑制剂与MDROs定植风险的关系。

研究收集截至2019年7月8日的相关资料,考察抑酸剂使用患者与未使用人群相比 MDRO定植风险差异。MDRO包括产生超广谱β-内酰胺酶、碳青霉烯酶或质粒介导的AmpCβ-内酰胺酶的肠杆菌、万古霉素耐药肠球菌、耐甲氧西林或耐万古霉素金黄色葡萄球菌,或多重耐药的假单胞菌不动杆菌。

26项观察性研究,包括29382名患者,其中11439名(38.9%)使用酸遏抑剂。初步荟萃分析结果表明,酸抑制使肠内肠杆菌和万古霉素耐药肠球菌的MDROs的肠定植风险增加约75%(OR=1.74;I2=68%)。异质性的部分原因是研究环境和酸抑制类型变化。

研究认为,酸抑制增加多药耐药微生物定植风险。鉴于全球抗微生物耐药性威胁,减少不必要抑酸剂使用是十分迫切的。
