天津医科大学总医院、天津市功能影像重点实验室于春水教授课题组最近报道了重度抑郁症遗传风险预测遗忘型轻度认知障碍患者向阿尔兹海默症转换的神经生物学机制,相关研究成果发表在《Brain》,题目为“Neurobiological substrates underlying the effect of genomic risk for depression on the conversion of amnestic mild cognitive impairment”。

遗忘型轻度认知障碍(aMCI)是一种认知缺陷状态,其严重程度不足以满足阿尔兹海默症(AD)的诊断标准,但是进展为AD的可能性极高。因此,在AD的早期预防过程中,确定能够预测从aMCI向AD转换的生物学标志是具有及其重要的临床意义的。其中,重度抑郁症(MDD)病史、抑郁症状、MDD并存诊断等均被证实可以增加aMCI向AD的转换风险。

该课题组通过临床样本的大数据分析发现MDD多基因风险评分(PRSMDD)可以预测aMCI向AD的转换风险。为了排除PRSMDD对aMCI转化的预测作用是由MDD和AD共同遗传变异所导致的可能性,此研究去除这两种疾病的常见遗传变异,构建MDD特异性遗传变异风险评分(PRSsMDD),发现PRSsMDD依然能够独立显着预测aMCI向AD的转换,并且左侧海马体积可以介导PRSsMDD与aMCI患者转换之间的因果关系链。同时,在PRSsMDD等级分组中,高分PRSsMDD组aMCI患者10年转换率(65.38%)比低分PRSsMDD组aMCI患者(49.13%)高出16.25%。

该课题组进一步探索上述预测效应背后的神经生物学机制。首先,使用两种生物信息学方法将构成PRSSMDD的遗传变异精细定位到基因中,基因富集分析和蛋白质-蛋白质相互作用网络分析均表明这些基因主要与神经元发育过程有关,并且参与淀粉样蛋白-β蛋白与其前体之间的分子结合,而这正是众所周知的AD患病的神经病理学机制。其次,本研究进一步证实这些基因在海马组织中主要于胎儿早中阶段发育期间具有时间特异性表达,并且主要在海马锥体神经元和中间神经元中具有细胞特异性表达。

本研究不仅发现PRSsMDD可以独立预测aMCI向AD的转换,PRSsMDD和PRSsAD的联合使用可以筛选出具有高度转换风险的aMCI患者,而且进一步证实预测背后的神经生物学机制,为AD患者的检测和早期预防提供了潜在的生物学靶标。
