研究认为达克珠单抗治疗可能导致神经学恶化及药物相关的嗜酸性粒细胞增多不良事件,可能导致新发自身免疫性疾病、肝脏衰竭及死亡风险。由于其潜在的严重副作用,欧盟以暂停达克珠单抗的应用

研究人员报告2例多发性硬化症(MS)患者经2次和4次皮下注射达克珠单抗后,出现严重神经功能恶化、药物相关的嗜酸性粒细胞增多的药物反应及全身症状(DRESS)。

本报告包括临床、MRI和组织病理学资料。

达克珠单抗为人源化白细胞介素-2受体单克隆抗体,用于复发性MS的治疗。DRESS为药物免疫反应,其特点是嗜酸性粒细胞增加、皮肤和内脏表现。达克珠单抗治疗后,2名患者出现暴发性神经功能恶化,同时伴有嗜酸性粒细胞增多和皮肤改变,符合DRESS症状。MRI表现为多个钆增强的幕上和幕下病变,包含基底节区、中脑区和小脑区。脑活检显示有明显的炎性浸润,包括大量嗜酸性粒细胞浸润脱髓鞘病变,这一特征对于MS而言不典型的,但与DRESS相符。此外,浆细胞数量及形态改变符合血管炎症状。
