Lancet:心血管死亡与水果、蔬菜和豆类饮食

研究认为,水果、蔬菜和豆类饮食可降低非心血管疾病和总死亡风险,每日摄入375-500克的水果、蔬菜和豆类即可获得最大的风险降低收益

近日研究人员在多个国家开展大规模研究,考察水果、蔬菜和豆类摄入与心血管事件及其死亡风险之间的关系。

PURE研究为一项大型前瞻性队列研究,共135335名35-70岁成人参与,参与者无心血管疾病,来自18个国家和地区,包括美国北部和南部,欧洲,中东,南亚,中国,东南亚以及非洲。研究人员考察参与者的饮食结构以及频率,采用标准化问卷收集有关人口因素、社会经济地位(教育、收入和就业)、生活方式(吸烟、体力活动和酒精摄入量)、健康史和用药情况以及心血管疾病家族史的信息。研究的主要终点是心血管疾病,包括心血管疾病死亡和非致命性心肌梗死、中风和心力衰竭,采用随机效应的Cox模型评估水果、蔬菜和豆类的饮食与心血管疾病事件和死亡率风险之间的关系。

研究持续了10年,总体而言,所有参与者每日摄入水果、蔬菜和豆类3.91次。在平均7.4年的随访期间,共发生4784起主要心血管事件,1649起心血管死亡以及5796 起死亡。多因素分析显示每日水果、蔬菜和豆类摄入量越高,主要心血管疾病(HR,0.90),心肌梗死(HR,0.99),心血管死亡(HR,0.73),中风(HR,0.92),非心血管疾病死亡率(HR,0.84)和总死亡率(HR,0.81)越低。研究发现每日3-4次水果、蔬菜和豆类摄入,患者的心血管事件风险最低(HR,0.78).单独分析表明,水果摄入与心血管、非心血管疾病和总死亡率的风险降低有关;豆类摄入与非心血管死亡和总死亡率降低相关;对蔬菜摄入而言,生蔬菜的摄入量与总死亡率的风险有着密切的关系,而熟蔬菜摄入量与死亡风险中等相关。

研究认为,水果、蔬菜和豆类饮食可降低非心血管疾病和总死亡风险,每日摄入375-500克的水果、蔬菜和豆类即可获得最大的风险降低收益。
