多囊卵巢综合征（PCOS）是青春期女孩中一种常见疾病，据称是由肝内脏脂肪过多引起的，并常常继发不育症和2型糖尿病。

近日，内分泌和代谢性疾病领域权威杂志Journal of Clinical Endocrinology & Metabolism上发表了一篇研究文章，研究人员旨在探究PCOS少女的基线miRNA谱，以及评估随机分组接受口服避孕药（OC）或螺内酯-吡格列酮-二甲双胍（SPIOMET，旨在减少肝内脏脂肪过多）治疗1年的效果。患者通过RNA测序对PCOS女孩进行miRNA谱评估，这些患者参与了一项随机、开放标签、单中心的预研究（n=31；年龄为15.7岁，BMI为23.1Kg/m2）。健康年龄和体重指数匹配的女孩（n=13）作为对照。通过qRT-PCR在整个研究人群中验证了差异表达的miRNA。通过检测PCOS女孩唾液孕激素来评估了治疗后的排卵率。

PCOS女孩循环miR-​​451a、miR-652-3p、miR-106b-5p和miR-206的浓度明显降低；通路富集分析表明，这些miRNA靶向参与能量稳态和细胞周期控制的基因。在本研究中，miR-451a以100％的敏感性和100％的特异性可诊断PCOS。SPOSMET（但非OC）可使PCOS女孩的miRNA正常化；OC或SPIOMET治疗1年后miR-451a浓度与治疗后排卵率密切相关（r=0.66; P<0.0001）。

由此可见，SPIOMET治疗1年可使PCOS女孩的miRNA谱正常化。循环中的miR-451a可能成为指导青春期PCOS诊断和治疗的生物标志物。
