Radiology:双源CT虚拟化平扫图像与肾脏占位性病变

背景:双源(DE)CT能够利用单期对比增强检查进行虚拟化非对比(VNC)图像重建,从而能够减少多期相CT检查来显示肾脏病变特征。然而,VNC图像评价肾脏病变效能的数据较少。本研究旨在验证利用VNC图像进行肾脏肿块CT检查是否能够显示肾脏病变及于CT平扫图像相比能够鉴别增强后非强化病变。

背景:双源(DE)CT能够利用单期对比增强检查进行虚拟化非对比(VNC)图像重建,从而能够减少多期相CT检查来显示肾脏病变特征。然而,VNC图像评价肾脏病变效能的数据较少。本研究旨在验证利用VNC图像进行肾脏肿块CT检查是否能够显示肾脏病变及于CT平扫图像相比能够鉴别增强后非强化病变。

本回顾性研究纳入了293例行患者,共有379处肾脏病变。所有患者行单能CT平扫检查,随后行肾实质期DE CT检查。利用特异性算法计算并重建VNC图像。利用VNC图像或平扫图像联合肾实质图像对病变进行分类。在VNC图像、CT平扫图像和肾实质期图像进行CT值测量。以非增强图像和病理学结果或随访24个月影像学结果为参考标准。

结果为,VNC图像与CT平扫图像对肾病变征象显示整体一致性较好(Cramer V= 0.85)。VNC图像在简化鉴别强化病变与非强化病变的诊断效能较高(ROC曲线下面积为0.91; 95%CI: 0.86, 0.95)。然而,VNC图像在显示增强肾病变的效果要较CT平扫图像诊断效能下降(ROC曲线下面积为0.91 [95% CI: 0.86, 0.95] vs 0.96 [95% CI: 0.93, 0.99]; P < .001)。VNC与CT平扫密度平均绝对差异为9.2 HU ± 8.7。

本研究表明,虚拟非增强图像能够准确显示肾脏病变征象,尽管对显示强化病变征象效能略下降。
