至少在最近20多年中,大量研究表明适度饮酒对于许多人而言具有保护心脏的作用,但其原因仍所知甚少。来自巴西圣保罗大学生物医学研究所的研究者曾在既往发表的一项动物研究中显示,急性乙醇给药后对小鼠心脏的保护作用是通过ALDH2激活而产生。在当前这项研究中,他们进一步明确了乙醛(一种来自乙醇代谢的中间产物)及其代谢酶(即ALDH2)在体外心脏缺血/再灌注损伤模型中的作用。

这项研究中使用携带人类失活点位突变ALDH2(E487K)的纯合子敲除小鼠(ALDH2*2),以及ALDH2的直接激活剂Alda-1,评估了乙醛对心脏的影响。ALDH2*2小鼠的乙醛清除功能受损,用于近似模拟人类中的相应表型。

结果显示,野生型(WT)小鼠的梗死面积与ALDH2*2小鼠相似。与乙醇诱导预处理相似,通过使用50μM乙醛进行预治疗可以增加ALDH2活性并降低WT小鼠中的心脏损伤,而不会影响心脏乙醛水平。然而,在接受乙醛预治疗ALDH2*2小鼠的心脏中,乙醛水平升高超过3倍,并且心脏缺血/再灌注损伤恶化。因此,外源性乙醛看起来对心脏缺血/再灌注损伤存在取决于ALDH2基因型的双峰效应。

这一研究项目的主要负责人,巴西圣保罗大学生物医学研究所的解剖学教授Julio Cesar Batista Ferreira总结指出:“我们的数据表明,适度暴露于乙醇会对心脏细胞造成轻微的压力,但不足以杀死它们。结果,细胞内的信号被重组,心脏细胞最终产生一种生化记忆来防止应激反应。这就是所谓的预处理,即当细胞处于更高的压力下时,它们将知道如何处理。”

然而,Ferreira教授还强调:“这一切都取决于人们的DNA。如果产生少量乙醇,消化乙醇产生的乙醛可以保护大多数人,但它也可以最大限度增加患有ALDH2基因突变的个体的心脏病发作所造成的损害。识别这些人很容易,例如,他们会在喝了一杯啤酒后变得满脸通红,抱怨头痛,并且他们对酒精的抵抗力并没有随着时间的推移而提高。”
