Schizophrenia Bulletin:心理所揭示精神分裂症与强迫症共病患者存在特殊的脑功能连接模式

精神分裂症一直被认为是最复杂的精神疾病之一,精神分裂症患者的认知、情绪和社会功能障碍在行为和神经层面均有表现。美国精神病学会最新发布的 DSM- 5 临床诊断手册强调精神分裂症谱系的概念,即把精神分裂症的发展过程看作一个具有不同行为和神经系统表现的连续体,并且这些表现会同样出现在其他相关精神障碍中。

精神分裂症一直被认为是最复杂的精神疾病之一,精神分裂症患者的认知、情绪和社会功能障碍在行为和神经层面均有表现。美国精神病学会最新发布的 DSM- 5 临床诊断手册强调精神分裂症谱系的概念,即把精神分裂症的发展过程看作一个具有不同行为和神经系统表现的连续体,并且这些表现会同样出现在其他相关精神障碍中。

临床研究发现精神分裂症患者更有可能共患其他精神障碍,如强迫症等,并且精神分裂症和强迫症共病患者具有独特的脑结构和功能改变、预后更差。目前共病患者的神经机制研究很有限,未有研究直接对比共病患者与精神分裂症、强迫症患者及健康人群之间的差异。这三类患者的脑功能连接是否不同仍然未知。

为填补这项空白,中国科学院心理研究所心理健康重点实验室、神经心理和应用认知神经科学(NACN)实验室研究员陈楚侨及其团队,与国际学者合作,揭示了这三类患者在静息状态下默认网络子脑区与全脑体素功能连接模式的不同变化。该研究纳入 22 名精神分裂症强迫症共病患者,20 名精神分裂症患者,22 名强迫症患者和 22 名健康被试,并分析了这些参与者静息状态下的功能连接模式。

研究表明,与其他三组相比,共病患者默认网络子脑区间表现出最高的功能连接强度,而在默认网络和突显网络子脑区间内表现出最低的功能连接强度。此外,与健康组相比,精神分裂症、强迫症及共病患者在默认网络和执行控制网络子脑区之间都表现出增强的功能连接;共病患者和精神分裂症患者均发现默认网络子脑区与颞中回之间的功能连接增强,但强迫症患者表现相反。这些发现为临床医生和研究人员提供了了解精神分裂症、强迫症和共病患者脑功能连接变化异同点的重要信息,特别是共病患者默认网络功能连接变化的特异性,并提出了针对不同精神障碍脑功能变化特点的新见解。目前,该研究团队正在研究一套认知、情绪以及基于任务的功能性神经影像测量方法,以系统地对比考察不同精神障碍患者。这些研究成果将为精神分裂症谱系和强迫症的临床诊断干预及其相关的精准脑科学方法提供指导。

研究工作得到了国家重点研究发展规划项目、北京市科学与技术基金、北京市科学与技术领军人才项目、中科院战略先导科技专项以及中科院心理研究所心理健康重点实验室的资助,相关研究成果发表在 Schizophrenia Bulletin 上。
