龈沟内,存在适当的有效和平衡的固有免疫反应以拮抗龈下菌斑微生物对于维持免疫平衡十分重要。这篇研究的目的是为了调查健康牙龈和牙周炎组织中的记忆T细胞亚群。

研究通过免疫组化检测了健康牙龈及牙周炎组织中T细胞(CD3+ , CD4+ , and CD8+ )的解剖位置。使用一组单克隆抗体混合剂(anti-CD69, anti-CD103, anti-CD45RA, anti-CCR7, anti-CD28, and anti-CD95)通过流式细胞仪技术检测所分离的牙龈细胞中记忆T细胞亚群。同时,也通过细胞内因子染色检测牙周炎组织中记忆T细胞的IL-17和IFN-γ表达。

结果显示,健康牙龈中含有2种记忆T细胞种群:1种为CD69-再循环种群,1种为CD69+牙龈固有记忆T细胞种群。具有过度记忆表型(CD45RA- CCR7- CD28+ CD95+ )的CD4+细胞组成了这2种种群的主要亚群。与健康牙龈相比,牙周炎组织中可观察到明显增多的CD4+ CD69+ CD103-记忆T细胞。牙周炎组织中的CD4+记忆T细胞表达IL-17或IFN-γ,而CD8+记忆T细胞仅表达IFN-γ。

结论:再循环和牙龈固有记忆T细胞代表着结缔组织中免疫监控网络的重要部分,以维持牙周的动态平衡。龈下菌落的失衡会损伤牙龈屏障,促使细菌抗原到达更深层的结缔组织内,进而激活记忆T细胞导致有害的炎症反应;标志着牙周炎的发生。
