影像组学的前沿研究与未来挑战

由于成像技术的迅速发展,医学影像已成为疾病管理中的重要模式,为临床提供了全面的视角和丰富的信息,在疾病筛查、早期诊断、治疗选择和预后评估等方面发挥着举足轻重的作用。现已知病灶形态或功能上的变化是由患者个体的基因、细胞、生理微环境、生活习惯和生存大环境等诸多因素共同决定的。若在常规影像学诊断基础上,通过深度挖掘数据,寻找出疾病的内涵特征,从而反映人体组织、细胞和基因水平的变化,将会对临床医学产生重大影响。基于这一理论,影像组学(radiomics)应运而生。它从医学影像中提取高通量特征来量化肿瘤等重大疾病,在肿瘤表型分型、治疗方案选择和预后分析等方面表现出巨大优势,是临床医学和生物医学工程的研究热点。本文系统梳理影像组学历史,从多方面论述这一学科的应用和发展。

1.影像组学概念及方法框架

2012年,荷兰学者Lambin首次提出影像组学概念,其思想来源于肿瘤异质性。实体肿瘤在基因、蛋白质、细胞、微环境、组织和器官层面上表现出的空间与时间的异质性,使病理学和分子学等有创检测方法结果的准确性及代表性受到限制。医学影像可全面、无创、定量观察肿瘤整体形态,对肿瘤的发展过程和治疗反应随时进行监测,从而为肿瘤异质性问题提供了可靠的解决方案;同时,影像组学假设微观层面的基因或蛋白质模式改变可在宏观影像学特征上有所表达。因此,Lambin认为影像组学为“高通量地从放射影像中提取大量特征,采用自动或半自动分析方法将影像学数据转化为具有高分辨率的可挖掘数据空间”。Kumar等进一步扩展,将影像组学定义为“高通量地从CT、MRI和正电子发射型计算机断层显像(positron emissiontomography,PET)中提取并分析大量高级的定量影像学特征”。Doroshow等在Nature ReviewsClinical Oncology发表文章,指出影像组学是转化医学未来发展方向之一。2014年,北美放射学会(Radiological Society of North America,RSNA)峰会主题即为“Radiomics: From Clinical Imagesto Omics”。Gillies在大会主题报告中提及,通过对影像的深入分析可量化微环境,预测肿瘤遗传异质性的程度。他认为,相较于传统的临床医学仅仅从视觉层面解读医学影像,影像组学可深入挖掘图像的生物学本质并提供临床决策支持。近年来,越来越多的学者关注影像组学,在肿瘤诊断、治疗和预后等方面进行探索,取得了大量成果,加快了肿瘤学的临床和转化研究。

影像组学方法框架一般分为5个部分:① 高质量标准化影像学数据获取;② 手动或自动图像分割与重建;③ 高通量特征提取与筛选;④临床预测模型建立;⑤ 构建共享数据库。影像组学是一种大数据分析方法,其研究结果必须在多中心进行验证,对数据的标准化、算法的可重复性和可靠性提出了很高要求,故框架中每一部分都极具挑战。

2.影像组学的临床应用

影像组学通过从不同模态影像中提取高通量特征并加以数据挖掘,可用于肿瘤分子分型、鉴别诊断、治疗方案选择、疗效检测和预后评估等多方面。目前,其在肺癌、头颈癌、乳腺癌、脑肿瘤、直肠癌、食管癌、前列腺癌、肝癌等多种肿瘤疾病中开展了初步探索。

2.1基因、分子标记和病理分型

影像组学认为,肿瘤宏观影像特征与微观基因、蛋白质和分子改变息息相关。2007年,Segal等在Nature Biotechnology发表文章,提出CT影像学特征与原发性肝癌全基因表达之间存在相关性,28个影像学特征可重建78%的基因信息,进而预测肝癌的细胞增殖、肝脏的合成功能和患者的预后,因此肝癌的基因活动可被影像学无损解码。Yoon等通过对539例肺腺癌患者进行研究,寻找CT和PET影像学特征与间变性淋巴瘤激酶(anaplastic lymphoma kinase,ALK)/c-ros原癌基因1酪氨酸激酶(c-ros oncogene 1 receptortyrosine kinase,ROS1)/原癌基因RET (rearranged during transfection proto-oncogene)表达模式之间的关系,发现这些影像学特征在ALK基因阳性与ROS/RET融合基因阳性组之间有显著差异。Gevaert等使用CT影像组学特征预测非小细胞肺癌(non-small cell lung cancer,NSCLC)中突变型表皮生长因子受体(epidermal growthfactor receptor,EGFR),受试者工作特征(receiver operating characteristic,ROC)曲线的曲线下面积(area under curve, AUC)为0.89。其中肺气肿、气道畸形与EGFR野生型有关,而毛玻璃样变预示EGFR突变。Yu等研究脑胶质瘤MRI图像特征与异柠檬酸脱氢酶1 (isocitrate dehydrogenase 1,IDH1)基因突变的关系,在110例二级脑胶质瘤患者术前MRI图像中提取671个高通量特征,筛选出110个高相关特征并建立预测模型,获得80%的IDH1分类准确率。Dang等使用MRI纹理特征预测头颈部鳞状细胞癌的肿瘤抑制蛋白p53,也获得81.3%的准确率。在乳腺癌方面,Wan等和Li等对动态增强MRI (dynamic contrast-enhanced MRI,DCE-MRI)图像开展研究,提取大小、形状、边界、纹理和动力学等特征,分析其与MammaPrint、Oncotype DX等基因检测系统的关联,预测乳腺癌复发。Zhu等分析了DCE-MRI图像特征与基因突变、微核酸表达、蛋白表达、基因通路表达、复制数变异等肿瘤微观结构的关系。

在分子分型方面,Wu等研究影像组学特征与肺癌组织学亚型(腺癌和鳞状细胞癌)的关系,从350例患者术前CT图像中提取440个形状、大小、灰度和纹理特征,单因素分析发现53个特征与肿瘤组织学显著相关;多因素分析中,特征筛选后由5个最相关特征建立的分类模型所得AUC为0.72。Yang等从MRI T1WI和T2抑水像(T2 fluid-attenuated inversion recovery,T2Flair)横截面、矢状面和冠状面图像中提取了5类共976个纹理特征,采用随机森林分类器进行脑胶质瘤分子分型和12个月生存期预测。Li等提取定量化MRI图像特征,进行浸润性乳腺癌的分子分型。研究发现,侵袭性强的肿瘤较大,内部回声不均匀,增强纹理中熵特征与分子分型相关性更大。进一步,Guo等使用影像组学与基因组学联合预测浸润性乳腺癌的表型,包括病理分级、淋巴结转移和免疫组织化学指标,共提取38个影像组学特征和144个基因组学特征,结果表明基因组学在免疫组织化学指标预测方面表现优异,影像组学更适用于预测病理分级,两种组学的联合应用并没有显著增加预测率。

2.2诊断与鉴别诊断

影像组学是大数据技术与医学影像辅助诊断的有机融合。传统的计算机辅助诊断方法多用于肿瘤筛查和鉴别诊断,而增加了高通量特征和数据挖掘的影像组学方法将有效提高诊断准确率。

Pham等从271例肺癌患者CT图像中提取两类纹理特征,实现了纵隔淋巴结良恶性鉴别,AUC为0.89,灵敏度为75%,特异度为90%。有研究对肺部图像影像数据库(The Lung ImageDatabase Consortium,LIDC)中的CT图像进行高通量特征提取,构建肺癌影像组学预测模型,用于肺癌良恶性评估。进一步,He等对240例孤立性肺结节患者分别采用普通CT和增强CT进行扫查,比较对比剂、重建层厚和卷积核对诊断的影响。结果表明,普通CT扫描、1.25 mm薄层CT和标准卷积核可为孤立性肺结节的诊断提供更多有价值的信息。在头颈肿瘤方面,Brown等将影像组学用于甲状腺结节良恶性鉴别,从多中心采集26例患者的扩散加权成像(diffusion weightedimaging,DWI)数据,从表观扩散系数(apparentdiffusion coefficient,ADC)图像中提取21个纹理特征,建立线性判别分析模型,分类准确率、灵敏度和特异度均超过90%。Fruehwald-Pallama等也将DWI纹理特征用于腮腺肿块的识别。Park等从DCE-MRI参数图中容积转运常数(Ktrans)、速率常数(Kep)和血管外细胞外容积分数(Ve)提取直方图特征用于识别口咽鳞状细胞癌和恶性淋巴瘤,两种癌症的Ktrans直方图中值和众数,以及Ve直方图的众数、偏度和峰度差异较大。Ve峰度是最有效的识别特征(准确率86%,灵敏度83%,特异度90%)。

Cameron等和Khalvati等分别在多参数MRI中提取影像组学纹理特征,实现前列腺癌的自动检测,准确率达87%和88%。Khalvati等的研究进一步表明,增加相关扩散成像(correlated diffusion imaging,CD)和高b值DWI (computed high-b DWI,CHBDWI)图像可有效提高检测的灵敏度和特异度。Wibmer等研究发现,从MRI图像提取灰度共生矩阵(gray level co-occurrence matrix,GLCM)特征有助于前列腺癌外周带与移行带的识别,并可用于Gleason评分。在T2WI和ADC图像上,外周带与高熵值、高惰性值、低能量值、低相关性和低均匀性有关(P<0.000 1~0.008)。

移行带与ADC图像特征(P<0.000 1)及T2WI图像的相关性(P=0.041)和惰性值(P=0.001)强相关,而Gleason评分与高熵值和低能量值有关。Litjens等通过研究70例前列腺全切患者术前多参数MRI图像,发现CHB-DWI鉴别前列腺癌与良性增生更有效,DCE-MRI可用于鉴别前列腺癌与前列腺萎缩或炎症,且ADC是诊断高级别前列腺癌最有效的特征。同样,影像组学也可在DCE-MRI中识别三阴性乳腺癌,将肿瘤区域特征与背景实质强化特征相结合后,获得的识别准确率AUC可达0.88。Hassan等在功能MRI图像中提取与异质性相关的纹理特征,鉴别脑部真正的功能活动区域。GLCM的不相关系数、熵、和方差、熵差值与脑功能相关性最强,回归模型的识别准确率高达80.19%。

2.3临床决策和疗效监测

不同于传统的医学影像辅助诊断,影像组学基于数据分析的方法从大量医学图像中挖掘出图像特征作为新的生物标记,有助于临床选择合适的治疗方案并监测治疗效果。

Shiradkar等提出基于多参数MRI影像组学特征和形变配准模型架构用于前列腺癌局部靶向放疗方案的精准制订。Lopez等研究MRI和磁共振波谱(magnetic resonance spectroscopy, MRS)影像组学特征与临床靶向体积和代谢体积的相互关系时,发现N-乙酰天冬氨酸水平异常的脑胶质瘤代谢肿瘤体积与MRI特征密切相关。Cunliffe等研究食管癌放疗患者CT数据,发现通过影像组学提取的纹理特征值与放射性肺炎的发生密切相关。

Huynh等、Hunter等和Mattonen等均使用影像组学方法定量评估肺癌放疗疗效。Huynh等从113例Ⅰ~Ⅱ级NSCLC患者治疗前CT图像中提取1 605个影像组学特征与常规影像学特征,发现小波分解特征与远处转移预后有明显相关性,整体生存期与常规影像学特征和影像组学特征均有关。Hunter等通过定量图像特征模型分析NSCLC患者放疗前CT影像资料,成功预测放疗后肿瘤退缩情况,为肿瘤的治疗评估和生存率预测提出了新的指标。Mattonen等开展了肿瘤放疗专家定性诊断与影像组学定量特征在肺癌放疗效果和复发预测中的比较,结果证明影像组学预测在整个随访期内与临床医师评价基本吻合,但在术后6个月内可早期发现临床医师难以发现的不典型复发病灶。Aerts等定量比较早期NSCLC患者治疗前后的CT图像,用于评价吉非替尼(Gefitinib)新辅助疗法的疗效。类似地,Antunes等在PET/MRI融合图像中开展研究,早期评估靶向药物舒尼替尼治疗转移性肾透明细胞癌的疗效。基于计算机提取乳腺癌DCEMRI图像特征发现,纹理分析可为临床医师在乳腺癌新辅助化疗开始前预测疗效提供更为准确的信息。Yip等研究基于肿瘤区域配准的化疗前后PET/CT纹理特征分析,预测了食管癌化疗效果(AUC>0.70)。Scalco等使用CT图像纹理特征研究头颈癌放疗中腮腺结构和功能的改变,发现放疗后CT密度均值和分形维数明显增加,基于体积和分形维数特征预测腮腺缩小的准确率为71.4%。Jansen等基于DCE-MRI药代动力学模型参数图的影像学特征,评估头颈部鳞状细胞癌放化疗效果,纹理分析表明放化疗可减少肿瘤异质性。夏凡等探索运用影像组学方法评估肝脏组织特征以预测化疗后肝功能异常。

Huang等回顾性分析326例结肠癌患者CT图像,建立影像组学模型预测结肠癌淋巴结转移的概率,一致性指数(conformity index,CI)为0.736。Vallières等使用氟脱氧葡萄糖 (fluorodeoxyglucose,FDG)-PET和MRI纹理特征预测软组织肿瘤的肺转移,通过逻辑回归多因素分析获得的FDG-PET/MRI融合图像的4个纹理特征预测效果最好(准确率98%,灵敏度95%,特异度93%)。Coroller等也将影像组学方法用于评价肺腺癌远处转移的可能性。

以上研究表明,影像组学可用于治疗方式的选择和临床疗效的监测,对多种癌症的个体化治疗方案制订有指导意义。

2.4预后预测

2014年,Nature Communication发表了Aerts等的研究,他们从1 019例肺或头颈部肿瘤患者CT数据中提取了440个量化肿瘤图像特征,包括灰度分布、形状和纹理等,这些影像学特征反映肿瘤异质性,与肿瘤病理类型、T分期、基因表达的模式相关。同时,这些特征在多个不典型肺及头颈部肿瘤数据库中显示出较好的预测预后价值,因此认为影像组学在肺癌及头颈部肿瘤中可识别预后表型。针对相同的数据库,Parmar等研究并寻找可靠的机器学习方法用于预测肺及头颈部肿瘤预后,以拓展影像组学在精准医学和肿瘤护理中的应用。

Song等研究认为,定量CT的表型特征可发现NSCLC的转移或侵袭能力,对预判肿瘤转归和指导临床个体化治疗具有重要价值。Emaminejad等将基因学生物标记核糖核苷酸还原酶M1(ribonucleotide reductasecatalytic subunit M1,RRM1)和切除修复交叉互补基因1(excision repair cross-complementationgroup 1,ERCC1)与CT影像组学特征联合应用,预测Ⅰ期NSCLC患者术后复发风险,AUC为0.84,显著高于两者独立预测的AUC。Coroller等使用影像组学方法从肺腺癌患者CT图像中提取635个特征预测远处转移和生存期,35个特征与远处转移相关,12个特征与生存期有关,其中LoG特征可显著提升预测的准确率。Paul等进一步通过卷积神经网络提取CT图像特征,对肺癌预后预测的准确率最高达82.5%。

Zhou等发现,在不同生存率亚组之间脑胶质瘤MRI区域改变有显著差异。Yang等分析脑胶质瘤的T1WI和T2 Flair图像,找到5种纹理特征,认为可预测脑胶质瘤分子亚型(AUC=0.72)和1年生存率(AUC=0.69),并能取代现有的有创活检。Cui等通过多区域图像定量分析方法,确定了量化肿瘤表面积和强度分布的5个影像标记,并证实具有预测脑胶质瘤预后的能力(CI=0.67,P=0.018),高于传统预测模型。RoisVelazquez等比较了109例脑胶质瘤MRI图像自动分割与医师手动分割的体积吻合度,并分析了与伦勃朗视觉感受图像(visually accessible rembrandt images,VASARI)特征和预后的关联性。

Li等认为,MRI定量影像组学特征联合多种基因检测可有效评估乳腺癌的复发风险。Vignati等从T2WI图像中提取GLCM的对比度和均匀性特性,比较其与传统ADC参数对前列腺癌生物侵袭性预测的效果。结果显示,GLCM特征比传统ADC参数与病理Gleason评分的相关性更强,可有效预测低风险的前列腺癌。Zhang等研究72例经过TPF(多西他赛+顺铂+氟尿嘧啶)诱导化疗的晚期头颈部鳞状细胞癌患者,发现肿瘤大小、淋巴结转移、临床变量、CT纹理和直方图特征与患者的总生存率相关。

Leijenaar等通过研究,发现并验证542例口咽鳞状细胞癌患者治疗前CT图像的一些影像组学特征,具有较好的预测预后价值,且不受CT伪影的影响。Chicklore等认为,PET/CT融合成像也可通过影像组学的纹理特征分析来预测部分肿瘤的治疗反应和预后。

 

3.影像组学在超声中的初步探索

CT、MRI和PET具有标准化的扫查协议,更易于进行医学大数据分析。Lambin等和Kumar等定义影像组学为从这3种成像技术中提取高通量特征以开展肿瘤异质性的研究,因此上述所有研究均基于CT、MRI、PET及其融合成像。相比以上3种成像技术,超声因无创无辐射、操作简单、快速成像和价格便宜等优势,在我国各级医院中广泛用于临床诊疗工作,在甲状腺、乳腺和腹部的早期检查中尤为重要。因此,若将影像组学方法扩展至超声图像,对疾病的早期诊断和预后预测将具有极大的价值。

周世崇等对77例甲状腺乳头状癌单灶患者的超声图像开展研究,分析病灶超声特征与淋巴结转移之间的关系,发现14个高指向性超声特征,联合预测淋巴结转移的准确率为73%,灵敏度为71%,特异度为74%,优于常规研究中二维超声扫查的诊断率,也高于一般报道的20%~31%的超声检出率。Qiao等设计了基于影像组学特征的定量化乳腺影像报告和数据系统(Breast Imaging Reporting and Data System,BIRADS),用于B型二维超声图像中乳腺肿瘤良恶性鉴别,取得优异的鉴别效果(准确率93%,灵敏度94%,特异度93%)。同样,Zhang等在超声弹性图像中提取364个高通量特征用于乳腺肿瘤鉴别诊断,准确率为88%,灵敏度为86%, 特异度为89%。李佳伟等将超声影像组学用于预测浸润性乳腺癌激素受体表达,并与医师预测结果进行了比较。

以上是影像组学在超声中的初步尝试,结果提示超声影像组学的可行性。但超声波的直线传播特性和脉冲回波成像方式使得二维超声图像采集受探头大小和放置位置的影响,操作者依赖性较强,图像无法标准化。同时,超声图像中大量的斑点噪声部分掩盖了病灶真实信息,给图像分割和高通量特征提取带来困难。因此,影像组学在超声图像中的应用有待进一步研究。

4.影像组学发展趋势与挑战

4.1影像获取及标准化

基于大数据挖掘的影像组学方法对影像学数据的质量提出了严格的要求。超声、CT和MRI是目前肿瘤常规诊断手段,数据量庞大。但不同厂商的机器在图像获取、重建算法和参数设置方面有很大差异,缺乏统一标准;即使同一台设备,造影剂剂量、扫描层厚、脉冲序列、成像深度和增益等也会对图像产生影响。此外,多模态多参数技术使得同一种疾病可采用多种影像方式观察。医疗机构针对不同类型疾病的检查方式并无指南或共识。因此,要获取相同或相似参数的大影像数据库十分困难。

美国国立卫生研究院(National Institutes ofHealth,NIH)和国家癌症研究院(National CancerInstitute,NCI)通过与多个国家医疗机构合作,建立了标准化临床影像数据库,如LIDC、TheCancer Genome Atlas(TCGA)、The Cancer Imaging Archive(TCIA)等,涵盖肺部、脑部、乳腺、前列腺等重要器官,可用于影像组学研究。

4.2高通量特征的稳定性

定量描述病灶属性的高通量特征是影像组学的核心,大数据分析和多中心验证均需稳定和可重复的特征。常用特征包括大小、形态、边界、直方图、纹理、分形维数、小波变换等。有研究证实,定量化特征可反映图像的细微差别和更深层次的信息,并避免由人为经验带来的诊断误差,在鉴别诊断、疗效监控和预后预测方面有重要作用。

然而,由于医学影像设备缺乏统一的图像获取和成像算法标准,同一病灶通过不同设备采集而获得的图像差别很大,给基于灰度值的特征如直方图、纹理分析等带来影响。同时,特征提取的前提是病灶区域的准确分割。对于边界模糊不清的肿瘤,手动分割、计算机半自动分割和计算机全自动分割的结果存在明显差异,降低了基于大小、形态和边界等特征的稳定性。

Balagurunathan等、He等、Yang等、Zhao等和Fave等对平扫CT和增强CT图像开展特征稳定性研究,比较对比剂剂量、注射时间、成像方式、重建层厚、卷积核等对特征的影响,用于肺部肿瘤的影像组学分析。在MRI图像中,RiosVelazquez等采用手动和自动方法分割脑胶质瘤,两者计算获得的肿瘤体积高度吻合。Hu等研究399例乳腺癌患者的二维超声图像,探索超声仪器型号、仪器参数和图像分割算法对影像组学特征的影响,筛选出46个高重复性、非冗余的稳定特征。在另外138幅乳腺肿瘤超声图像上进行验证,良恶性识别效果优异(AUC=0.92,准确率为86%,灵敏度为90%,特异度为81%)。

4.3特征选择与建模

有限样本下用大量特征进行分类和预测,不仅计算时间长,效果也未必最优。数量庞大的高通量影像学特征提取后,需采用特征选择方法获得最佳性能表现的特征集,输入至准确可靠的机器学习算法或统计学途径建立分类或预测模型。

Parmar等研究发现,影像组学预测准确率主要受特征个数、特征筛选方法和分类器的影响。他从101例头颈癌患者CT图像中提取440个影像组学特征,比较14种特征选择算法和12个模式识别分类器对生存期的预测。结果发现,最小冗余最大相关方法(AUC=0.69,稳定性=0.66)、互信息特征筛选法(AUC=0.66,稳定性=0.69)和条件最大熵特征筛选法(AUC=0.68,稳定性=0.70)预后效果最好。分类器方面,贝叶斯分类器(AUC=0.67)、随机森林分类器(AUC=0.61)和最邻近分类器(AUC=0.62)取得了最佳预测结果。对NSLCL患者的分析也得到类似结果。因此,更准确、适用广泛的特征选择和模式识别方法是影像组学的突破点。

4.4多中心验证

现有的影像组学研究大多是单一机构的小样本探索,所得结论缺乏广泛验证。可预见,未来影像组学必须经过多中心、大样本、随机对照临床试验反复检验和提炼,才能准确、可靠、有效地指导临床医疗策略。不同地区的多中心研究能提供多样性样本资料,可更好地为影像组学训练集和验证集诠释肿瘤异质性,符合精准医学的发展需求。影像组学研究的核心是特征分析、提取和应用,但目前各项研究的特征之间缺乏足够的统一化和标准化,在一定程度上影响了研究结论的推广和使用,多中心联合研究通过协调与沟通,可很好地解决这一问题。根据循证医学要求,任何一种方式应用于临床都需经过完善检验,具备高质量证据。多中心临床研究是较为常见的方案,通过这样检验获得的证据,可有力地支持影像组学服务于临床诊疗工作。

5.总结与展望

影像组学通过从多模态影像中提取和挖掘大量特征,定量分析隐含在医学图像背后的人体分子与基因变化,为解决肿瘤异质性这一难题提供了思路,已在高发肿瘤疾病的诊断、治疗和预后方面取得了可靠结果。作为一种新兴研究方法,尚有许多关键科学问题和技术有待进一步探索。相信随着医学影像学数据的不断积累和标准化,以及各类图像分割、特征提取、特征选择和模式识别方法的迅速发展,影像组学将会对临床医学产生深远的影响和巨大的变革。
