Blood:镰状细胞病(SCD)的血管自主神经功能异常

热刺激时,SCD患者的血管收缩反应更快更强,表明他们的微血管存在自主神经过敏;反复热刺激的累积性血管收缩逐渐减少血流灌注,微血管红细胞转运减慢。

镰状细胞病(SCD)的受试者在实验环境中表现出对冷和热感觉的主观超敏,如冷暴露等已知诱因引起血管闭塞危象的潜在机制尚不明确。微血管血流量(MBF)减少增加了镰状红细胞在微血管系统中被困的可能性,从而增加了血管闭塞的风险。由于SCD受试者有自主神经功能障碍(dysautonomia),Veluswamy等人推测热暴露会导致他们的微血管系统自主神经过敏,从而增加血管收缩的倾向。

因此,Veluswamy等人将17名SCD和16名对照组受试者暴露在一系列预定的阈值温度下,通过放置在右手上的热模式来检测冷和热以及冷/热疼痛。用光容积描记仪测量对侧的MBF,并用心率变异性评估心脏自主神经平衡。

结果发现,予以在检测阈值和痛阈值强度的温度刺激后,几秒钟内均会导致对侧手的MBF显著降低,其中SCD受试者表现出更明显的血管收缩(p=0.019)。此外,与对照组相比,慢性阻塞性肺疾病受试者表现出更严重的进行性血流减少,热刺激间歇期恢复较差(p=0.042)。SCD受试者的血管收缩速度比对照组更快(p=0.033),特别是在冷检测刺激下;焦虑程度越高的个体血管收缩速度也越快(p=0.007)。
