循环系统中的循环肿瘤细胞(CTC)空间异质性及分子特征尚不清楚。CLIN CANCER RES近期发表了一篇文章,作者研究了肝细胞癌转移路线中的CTC的分布特点和生物学特征。

研究纳入了73例局部肝细胞癌患者,在肿瘤切除前收集外周静脉(PV),外周动脉(PA),肝静脉(HV),肝下腔静脉(IHIVC)和门静脉中的血液,分析CTC的上皮间质转化(EMT)亚型。评估不同血管区的CTC的临床意义。研究结果表明,肿瘤输出血管和外周血管间的CTC数量和大小梯度明显。对CTC的跟踪检测表明在扩散过程中,其呈现出聚集-游离-聚集特征。单细胞特征检测发现不同血管区CTC的EMT呈现出异质性。在释放时CTC主要为上皮性,但是在血液中通过Smas2和β-catenin相关信号通路转化为EMT活化亚型。原发肿瘤EMT活化与HV中总CTC数量有关,而不是CTC的上皮或EMT活化亚群。随访分析表明,肝静脉和外周循环中CTC和循环肿瘤微栓负荷可以预测术后肺转移和肝内复发。

文章最后认为,目前数据说明循环系统中CTC存在细胞分布和生物学特征的空间异质性。多血管CTC检测有助于揭示转移机制及预测肝细胞癌患者术后复发和转移。
