结果:在3439954人年的随访中,发生了33304例死亡。已知和可疑危险因素调整后,总膳食脂肪与碳水化合物总量相比,与总死亡率呈负相关(最高和最低五分之一组相比较,HR=0.84; 95% CI, 0.81-0.88; 趋势P值 < .001)。具体的膳食脂肪与总死亡率的关系如下:饱和脂肪酸 HR=1.08 (95% CI, 1.03-1.14);多不饱和脂肪酸(PUFA) HR=0.81 (95% CI, 0.78-0.84);单不饱和脂肪酸(MUFA) HR=0.89 (95% CI, 0.84-0.94);反式脂肪 HR=1.13 (95% CI, 1.07-1.18)(所有趋势P值 < .001)。使用PUFA和MUFA替换5%的来自饱和脂肪的能量,可以分别降低27%的总死亡率(HR, 0.73; 95% CI, 0.70-0.77)和13%的总死亡率(HR, 0.87; 95% CI, 0.82-0.93)。ω-6 PUFA最高和最低五分之一组相比较,总死亡率HR=0.85 (95% CI, 0.81-0.89; 趋势P 值< .001)。摄入ω-6 PUFA ,尤其是亚油酸,与死亡率呈负相关;而海洋ω-3 PUFA 的摄入对总死亡率的降低效果很轻微 (最高和最低五分之一组相比较,HR=0.96; 95% CI, 0.93-1.00; 趋势P值 = .002)。

结论:不同类型的饮食脂肪与总死亡率和特定死亡率有不同的关联。这些研究结果支持目前的饮食建议,使用不饱和脂肪取代饱和脂肪和反式脂肪。
