J Endod:光敏化功能化纳米粒子在脂多糖污染根管内的体内生物活性评估

消毒根管内牙本质边界持久存在的脂多糖(LPS)会影响牙髓治疗的效果。这篇研究通过骨内植入体模型并以新组织形成作为标志点,评估光敏化玫瑰红功能化壳聚糖纳米粒子(CSRBnps)对LPS污染的根部牙本质的体内影响。

研究将55例人的3-mm长牙根的1.2-mm根管管腔分为5组(n=10):组1,根管未污染;组2,根管被铜绿假单胞菌LPS污染;组3,污染根管使用NaOCl消毒;组4,感染根管使用NaOCl和氢氧化钙消毒;组5,污染根管使用NaOCl和光敏化(λ=540nm, 40J/cm2) CSRBnps (300μg/mL)消毒。随后,将样本植入豚鼠的下颌骨,4周后将样本块状解剖并对根管内容物进行组织学和免疫组化分析。在内生的新组织和牙本质界面(50μm)观察分析成纤维细胞、破骨细胞、炎症因子、牙本质吸收、矿化和血管生成(分为2型,1型:无感染和吸收,表明LPS无活性;2型:感染和吸收)情况。通过Fisher exact检验分析参数发生的频率。

结果显示,1型结果发生在组1和5,2型发生在组2,1型和2型混合发生在组3和4。另外,组1和组5(P>.05)的结果与组2、3和4显著不同(P<.05)。

结论:LPS污染的根管通过光敏化CSRBnps实施体内消毒可以促进新组织的内生,并且没有炎症或吸收的症状,表明它可以有效的失活牙本质边界的LPS。
