JCLA:酶联免疫吸附法与间接免疫荧光法诊断肺炎支原体感染的比较

本研究旨在比较酶联免疫吸附法(ELISA)和间接免疫荧光法(IFA)对肺炎支原体感染的诊断价值。 2016年3月至2017年5月,研究人员招募180例疑似肺炎支原体感染患者。采用ELISA法检测血清蛋白试剂盒,IFA法检测肺炎链球菌IgM抗体。采用Cohen’s kappa统计量评价ELISA与IFA测定结果的一致性,采用多元logistic回归分析评价ELISA与IFA测定结果不一致的危险

本研究旨在比较酶联免疫吸附法(ELISA)和间接免疫荧光法(IFA)对肺炎支原体感染诊断价值。

20163月至20175月,研究人员招募180例疑似肺炎支原体感染患者。采用ELISA法检测血清蛋白试剂盒,IFA法检测肺炎链球菌IgM抗体。采用Cohens kappa统计量评价ELISAIFA测定结果的一致性,采用多元logistic回归分析评价ELISAIFA测定结果不一致的危险因素。

研究对象平均年龄46.6±21.1岁。ELISA法和IFA法检测肺炎支原体感染阳性率分别为15.6% (95% CI: 11.0% 21.6%)10.0% (95% CI: 6.4% 15.3%)。总阳性率为19.4%(95%可信区间:14.3%25.8%)。多因素分析中ELISAIFA检测结果不一致的相关变量为:男性(OR: 0.366;95% CI: 0.149, 0.899;年龄(>33;OR: 0.313;95%置信区间:0.129,0.758;P < 0.05)

研究表明,在对肺炎支原体感染的检测中,ELISA法与IFA法的IgM检测结果一致性较差,女性和年龄偏小是导致不一致的重要危险因素。建议采用ELISAIFA联合检测,以便在临床实践中发现更多疑似肺炎支原体感染的患者。
