Radiology:扩散加权成像评价侵入性乳腺癌与无转移生存期关系的价值

本研究共纳入了258例患有侵入性乳腺癌并行术前MRI DWI成像的患者。由两名放射科医生在未知临床信息的情况下回顾性分析DWI图像。人工测量放置于病变内感兴趣区的平均ADC、最小ADC和最大ADC值,计算ADC值差异,评价肿瘤内部成分异质性。调整临床病理指标后,利用Cox比例危害模型验证ADC值与无远处转移生存期的相关性。

结果为,共有25/258例患者(9.7%)在随访51个月后出现远处转移。在有远处转移患者的ADC差值要明显高于无远处转移的患者(平均分别为0.743 × 10-3 mm2/sec vs 0.566 × 10-3 mm2/sec; P < .001)。多变量Cox比例危害模型分析,ADC差值越高(>0.698 × 10-3 mm2/sec) (HR, 4.5; 95% CI: 2.0, 10.0; P < .001)、出现前哨淋巴结转移(HR, 3.3; 95% CI: 1.2, 9.3; P = .02)和雌激素受体阴性(HR, 2.6; 95% CI: 1.2, 5.7; P = .02)与无远处转移生存期越差具有相关性。

本研究表明,扩散加权成像中表观扩散系数差值越高与侵入性乳腺癌患者无远处转移生存期越差具有相关性。
