研究过程中,体重指数< 30 kg/m218-35岁非肥胖成年人(N = 254)接受了稳定同位素标记的OGTT。采用口服最小模型测定胰岛素敏感性,葡萄糖有效性和细胞功能。根据OGTT期间的胰岛素反应分层为四分位数,四分位数1的反应最低,四分位数4的反应最高。

研究结果显示,13%的人空腹血糖受损(I FGn=14)或糖耐量受损(IGTn=19),允许在正常血糖和糖尿病前期范围内对胰岛素反应的连续体进行比较。体重指数(~24 kg/m2)在胰岛素四分位数上与IFGIGT组相似。尽管有相似的血糖变化,空腹胰岛素、三酰甘油和胆固醇在四分之一组升高。胰岛素敏感性在四分位4中最低,并伴有胰岛素分泌增加和胰岛素清除减少。IFG患者的胰岛素敏感性和细胞功能与四分位数23的患者相似,但比四分位数4的患者更敏感。虽然IGT患者的胰岛素抵抗程度与四分位4相似,但他们的细胞功能缺陷更为严重。血浆支链氨基酸在四分位4IFGIGT中没有升高。

研究结果表明,正常血糖的年轻非肥胖成人高胰岛素血症表现为胰岛素分泌增加和胰岛素清除减少。与IFG相比,个体表型特征显示高胰岛素血症患者更接近于IGT患者,这表明高胰岛素血症患者可能是IGT的连续体。此外,血浆支链氨基酸可能不是鉴别年轻非肥胖成年人高胰岛素血症和胰岛素抵抗的有效生物标志物。
