背景:C反应蛋白(CRP)和慢性阻塞性肺疾病(COPD)之间是否有因果关系并不清楚。作者使用孟德尔随机化方法评价了遗传性血清高CRP引起COPD的假设。方法:作者检测了血清高敏CRP,CRP基因型的四个单核苷酸多态性,在哥本哈根心脏研究7974例和哥本哈根普通人群研究32 652例中筛选出肺功能确定由于COPD住院的COPD。结果:血清CRP>3 mg/l者和<1 mg/l者患COPD

背景:C反应蛋白(CRP)和慢性阻塞性肺疾病(COPD)之间是否有因果关系并不清楚。作者使用孟德尔随机化方法评价了遗传性血清高CRP引起COPD的假设。

方法:作者检测了血清高敏CRP,CRP基因型的四个单核苷酸多态性,在哥本哈根心脏研究7974例和哥本哈根普通人群研究32 652例中筛选出肺功能确定由于COPD住院的COPD。

结果:血清CRP>3 mg/l者和<1 mg/l者患COPD的危险度为1.8和2.8,哥本哈根心脏研究中人群因COPD住院风险为1.6,而哥本哈根普通人群为1.8.四个CRP基因多态性基因组合与血清CRP升高62%有关。但是在两组人群刚中这些基因型组合与COPD风险和因COPD住院率并不相关。在工具变量分析中,双倍血清CRP与双倍遗传性高CRP导致COPD的相对危险度分别为1.27 (95% 可信区间1.25~1.30) 比1.01 (0.81~1.26),COPD住院的相对危险度为1.47 (1.43~1.51) 比0.82(0.59~1.13)。

结论:虽然CRP升高与COPD诊断和住院相关,遗传性血清高CRP与临床COPD无关,这表明CRP水平和COPD之间并没有因果关系。
