OCC 2020丨谁能拯救“蓝嘴唇”?肺动脉高压辩论会精彩角逐

2020年5月28日,由上海医学会和上海市医学会心血管病分会主办的第十四届东方心脏病学会议第一天。其中的肺循环疾病论坛,特设肺动脉高压(PAH)靶向药物治疗辩论赛环节,由多位专家针对两个主题进行辩论性质的深入交流,用一个又一个有力的例证证明己方观点的同时也在反驳对方观点,双方唇枪舌战、你来我往,精彩辩论赢得了全场的掌声。

众所周知,肺动脉高压患者需终生服药。近年来,随着对肺动脉高压发病机制认识的不断深入以及大量新型药物的研发,使得PH的治疗进入了多元化时代,现有的分子靶向药物在临床治疗上取得了一定疗效,但主要针对第一大类PH和第四大类CTEPH,对于第二大类左心疾病相关PH和第三大类肺部疾病所致PH,靶向药物治疗效果不明确,指南也未予推荐。因此靶向药物在这两大类患者中的应用存在争议,辩论也就此展开。

辩题一:左心病变肺动脉高压是否需要靶向药物治疗

正方一辩:靶向治疗可用于CpcPH

范粉灵教授主题汇报

来自西安交通大学第一医院的范粉灵教授指出,随着肺动脉高压机制研究的进展,靶向药物在混合性毛细血管前后肺高压(CpcPH)应用的治疗时代已经到来,她从理论依据、研究依据、实践依据三方面进行正方辩论。

首先,在理论上,近年来PH定义在变化,病理机制的认识在发展。从内皮功能紊乱到血管收缩再到血管重构致肺血管阻力增高,危险分层在发展,由单纯右心功能变为多因素评估。同样的,左心疾病相关肺高压(PH-LHD)的分类也在发展,第一大类PAH已经明确证实靶向药物可以改善预后。

其次,在研究方面,基于内皮稳态的PAH靶向治疗药物在LHD-PH中应用均有不同程度探索。目前的指南不推荐靶向药物治疗LHD-PH,依据在于前期的这些研究由于超声心动图刚应用,对HFrEF和HFpEF区分不清,对孤立性毛细血管后肺高血压(Ipc-PH)和Cpc-PH更是不清,研究者多为心衰专家,多数观察靶向药治疗心衰的结果,而非以LHD-PH为治疗目标,且无研究采用Ipc-PH与Cpc-PH区别诊治的研究设计。

最后,在实践中,尽管LHD-PH和PAH进展机制刚好相反,不论在小鼠还是患者中发现,与Ipc-PH相比,Cpc-PH造影结果显示肺血管类PAH重构样改变。总之,Cpc-PH介于LHD和PAH之间,基于以上分析认为靶向治疗可用于CpcPH。

反方一辩:左心病变肺高压不能用靶向治疗

来自上海交通大学医学院附属瑞金医院的杨震坤教授认为,对于LHD-PH,谨慎应用靶向药物,从定义、病理生理机制,分类回顾和靶向药物的治疗现状以及未来的研究方向几方面展开辩论。

首先,肺循环是一个高流量低阻力的循环,静息状态下大于25 mm Hg或运动状态大于30 mm Hg定义为肺高压。临床分类包括5大类,血流动力学分为3类。LHD-PH属于第二大类,占PH的48%-68%左右,随着疾病进展,一部分病人出现类PAH表现,出现肺动脉重塑,这是尝试用靶向药治疗LHD-PH的依据。LHD-PH的诊断不仅要看肺动脉压力,还要看DPG、肺血管阻力等,对于靶向药物应用的尝试要有目的性。

在理论进展的基础上,对于LHD-PH,2015年指南推荐原发病治疗,靶向药物仅为ⅢC类推荐。由于缺乏循证医学的证据支持,若盲目使用靶向药物,结果可能会适得其反。以往靶向药物治疗LHD-PH的临床研究中,总的结果令人失望。更多的探索研究仍在进行中,因此目前证据缺乏的情况下,特别是Ipc-PH还是单纯治疗原发病为主。

今后的研究方向应是LHD-PH患者首先筛查有无心功能不全,有无肺高压,分类是孤立的毛细血管后性肺高压,还是混合性肺高压,对于后者要有针对性的开展研究,积累经验,然后再拓展到左心疾病相关肺高压人群。希望今后开展RCT研究,以便确定哪些人群能从靶向药物获益。

正方二辩:靶向药物治疗可以用于混合型毛细血管前后肺高血压

来自南京医科大学第一附属医院的张海锋教授从Cpc-PH与PAH存在相似的血流动力学、相似的病理生理改变、Cpc-PH靶向治疗的证据三方面进行阐述。

首先,Cpc-PH与PAH存在相似的血流动力学改变,LHD-PH是最常见的肺高压,包括很多疾病,比如扩心病、瓣膜病等。这些病人若不去干预预后很差。临床上有些Cpc-PH的患者经过原发病手术治疗,肺毛压降低至正常,此时血流动力学和PAH完全一致,这部分患者应该筛选出来,靶向治疗可能是获益的。

其次,Cpc-PH与PAH存在相似的病理生理改变,开始时肺动脉压力被动升高,导致一系列神经体液因素改变,最后就导致肺血管重构引起Cpc-PH,和PAH相似。

最后,Cpc-PH靶向治疗有一些证据。首先治疗左心疾病包括心衰药物、介入治疗、纠正结构异常,能降低PAWP,靶向药物针对第一类PAH证据明确,但是LHD-PH临床患者众多,因此很多研究也在探索,研究结果有好有坏。究其原因,在于未能筛选出经左心疾病治疗后降低了肺毛压的这部分患者。总之,对于经过治疗,PAWP已降低的Cpc-PH,其病理生理已接近PAH,靶向治疗可能有效。

反方二辩:左心疾病相关肺动脉高压应用靶向药物副作用大,反对使用

徐大春教授主题汇报

来自同济大学附属第十人民医院的徐大春教授从概念、临床分类、血流动力学分型、病理学特征、临床研究荟萃、指南等方面发表反方辩词。

LHD-PH尚无准确定义,与左心疾病严重程度相关,患者往往年龄偏大,许多靶向药物副作用大。研究显示左心衰导致肺重量、密度增加,主要为肺纤维化改变和肺淤血,而非主要是肺动脉高压改变。之前的临床研究结果显示靶向药物前途渺茫,2015年ESC/ERS指南并不建议靶向药物用在左心疾病相关肺高压上,LHD-PH重点是在左心衰上,不应该舍弃左心系统疾病而去治疗肺高压。心衰治疗应在最优化的ACEI、ARB、β受体阻滞剂、螺内酯等的治疗上,再考虑肺高压治疗。

总之对于LHD-PH,肺动脉高压靶向药物不宜使用,相关副作用会适得其反,纠正左心衰才是根本。此外我们可以另辟蹊径,有研究能提示β受体激动剂沙丁胺醇吸入剂、肺动脉消融、呼吸机自动气道正压通气对减低肺动脉压力有帮助,有待未来研究的证实。  

南昌大学第一附属医院郑泽琪教授、天津医科大学第二医院张承宗教授在总结时称LHD-PH还是要做深入的分层,其中就包括左室射血分数保留、中间和降低的心力衰竭。HF值得关注,发病机理复杂,存在舒张和收缩功能不全,在HFpEF患者中,进一步右心导管区分Ipc-PH和Cpc-PH,对于没有肺高压的心衰和Ipc-PH不用靶向药,只有Cpc-PH这部分患者要加强循证医学研究。此外还有区分病因,是高血压造成的还是糖尿病肾功能不全引起的,这种进一步的分层可能对研究更有好处,还要考虑组织基因学的研究,进一步扩大循证医学的证据。

辩题二:低氧性肺高压or肺高压致低氧?

肺高压致低氧:

来自北京协和医院的蒋鑫教授首先从一个病例谈起。一名26岁女性,活动后气促,诊断为CTEPH,重度肺动脉高压,在美国行PEA+PFO修补,术后发生严重低氧,ECMO维持一周后恢复出院。术后复查氧合依然没有改善,之后病情进一步恶化,发展为严重紫绀,心功能Ⅳ级,肺动脉造影显示肺动脉血流缓慢。因此在此例肺动脉高压中,低氧主要是结果。

肺动脉高压导致低氧,可能通过以下因素:严重右心衰、左心衰肺淤血弥散功能降低、先天性心脏病右向左分流、CTEPH、PVOD/PCH、HHT等。此外,低氧性肺动脉收缩是导致PH发病和疾病进展的关键致病因素,对于合并II型呼衰患者首要排除COPD等肺部疾病。

PAH合并低氧的治疗,首先要强化靶向治疗,改善心脏舒张功能,CTEPH或肺动脉炎患者考虑外科和介入治疗。同时进行氧疗,指南推荐动脉氧分压小于60 mm Hg或氧饱和度小于90%进行家庭氧疗,但无循证医学证据,此外基础肺部疾病的对症治疗和呼吸支持治疗,对低氧为因的PH相当重要。

低氧致肺动脉高压:

来自同济大学附属肺科医院的王岚教授带来一例低氧相关PH的分享。患者男,57岁,活动后气促1年,加重1月,最终诊断为慢性肺部疾病相关PH(非严重型)。给予氧疗、扩张气道和利尿治疗。由于疫情耽误复查,二次入院时症状更严重,诊断为慢性肺部疾病相关PH(严重型),治疗上除了氧疗、扩张气道,还加用了肺动脉高压靶向药物,症状有所缓解,等待肺移植。

此病例印证了临床第三大类肺部疾病相关PH,低氧和肺动脉高压互为因果的概念。低氧可以引起内皮紊乱、肺血管收缩,最终引起肺血管重构、肺动脉高压。靶向药物治疗在肺部疾病相关PH颇有争议,有人认为光增加肺部血流,未能改善通气,增加了通气血流比例失调,会进一步加重肺部低氧。也有专家认为,低氧性肺高压晚期,积极改善肺血管,进一步改善右心功能,能改善患者症状,目前尚无确切的循证医学证据。

复旦大学附属华山医院李圣青教授总结时称,该病例展示了慢阻肺低氧血症Ⅱ型呼衰发生发展,直至需要肺移植的过程,需要和肺动脉高压引起的低氧血症鉴别。郑泽琪教授谈到低氧性肺高压还是肺高压致低氧这个话题两者很难分开,如高原性肺高压,在低氧环境下发生肺动脉高压,而临床好多疾病是由于肺动脉高压进展导致缺氧的加重,互相影响治疗上要根据病因采取针对性治疗。

本次论坛和辩论赛的设计者,上海交通大学医学院附属仁济医院心内科沈节艳教授全程参加了此次辩论。在谈到设置二场辩论赛的初衷时,她提到:左心病变肺高压和低氧性肺高压是临床最常见的肺高血压类型,前者临床上有滥用靶向药物倾向,而后者对低氧的正确析因才能引导正确的治疗,设置二场辩论旨在引导大家对这二种常见肺高压病的正确识别、精准施治。感动于线上各位专家的精心准备,从不同角度有理有据的讲解,胜负难辨,精彩绝伦。观点没有绝对正误,但沈教授认为左心病变肺高压更倾向于不接受靶向药物治疗,因为即使左心原发病的治疗纠正了肺毛压,左心的基础疾病仍潜在存在,在肺动脉扩张药物的应用下仍有可能加重肺淤血,这也是指南始终没有给出对LHD-PH进行靶向药物治疗的推荐的原因。上海十院的徐大春教授举证充分、观点明晰。关于肺高压致低氧和低氧性肺高压,二位专家的病例选择恰当,看待问题全面深刻,展示了深厚的知识功底。本次辩论也让每一位观众感受到思维的碰撞,对临床实践和临床研究有了更多的启示和理解。
