DDS: 血浆线粒体DNA分析有助于预测急性胰腺炎患者的胰腺坏死的发生风险

背景
在治疗急性胰腺炎(AP)中常常需要特异性血浆生物标志物来预测胰腺坏死(PNec)的情况。本项研究的目的就是探讨血浆线粒体DNA(mtDNA)能否对AP患者出现PNec有预测作用。

方法
研究人员前瞻性收集了从2015年6月至2017年6月出现症状的AP患者,并使用对比增强CT扫描评估PNec。使用qRT-PCR测量血浆mtDNA浓度。

结果
在74名AP患者中,研究人员发现在重度AP患者中血浆mtDNA的中位数水平显着高于轻度AP患者和健康对照,但在中-重度AP患者中未发现mtDNA水平有明显异常。PNec患者血浆mtDNA水平高于无PNec患者(774.2 [IQR 397.6-2205.0] vs 169.5 [IQR 73.6-683.4] pg / ml,P<0.05)。用于预测PNec的mtDNA的受试者操作特征曲线(ROC-AUC)下的面积高于CRP(0.813 [95%CI 0.705-0.895]对比0.678 [95%CI 0.558-0.783])。当使用302.5pg / ml作为截断值时,诊断PNec的灵敏度和特异性分别为90.9%和68.3%。最后,连续肾脏替代治疗后血浆mtDNA水平显着下降(717.7 [IQR 307.00-1370.00]对比237.5 [IQR 117.20-464.80] pg / ml,P<0.01)。

结论
与传统CRP相比,AP患者血浆mtDNA含量升高可作为PNec的更准确的早期预测因子。
