JCLA：循环CEA‐dNLR评分预测转移性胆囊癌患者的临床结果

癌症相关炎症可促进胆囊癌(mGBC)的发生和转移。循环炎症相关细胞和蛋白水平及其比值可能提示GBC患者慢性炎症的严重程度，且它们均是预测mGBC预后的候选生物标志物。 在我们的研究中，对220mGBC病人的循环免疫细胞、纤维蛋白原(Fib)、白蛋白(铝青铜),前白蛋白(pAlb)进行检测。然后计算中性粒细胞~淋巴细胞比率(NLR)派生的嗜中性粒细胞~量淋巴细胞比率(dNLR)、淋巴细胞高至单

癌症相关炎症可促进胆囊癌(mGBC)的发生和转移。循环炎症相关细胞和蛋白水平及其比值可能提示GBC患者慢性炎症的严重程度，且它们均是预测mGBC预后的候选生物标志物。

在我们的研究中，对220mGBC病人的循环免疫细胞、纤维蛋白原(Fib)、白蛋白(铝青铜),前白蛋白(pAlb)进行检测。然后计算中性粒细胞~淋巴细胞比率(NLR)派生的嗜中性粒细胞~量淋巴细胞比率(dNLR)、淋巴细胞高至单核细胞比率(LMR)、血小板量至淋巴细胞比率(PLR)、铝青铜~ Fib应承担的比例(误判率)和心房纤颤~ pAlb比率检测。我们对这些患者进行了3年的随访，并使用X‐tile软件、Kaplan‐Meier曲线、Cox回归和时间依赖的受试者操作特征(ROC)调查了这些生物标志物与这些患者3年总体生存率(OS)之间的可能关联。

研究结果显示，pAlb和LMR高的患者的OS明显优于生物标志物低的患者。然而，CEA、dNLR和FPR高的患者存活率明显低于这些生物标志物水平低的患者。CEA和dNLR随时间变化的ROC曲线下面积(AUC)分别高于pAlb、LMR和FPR。此外，更高的CEA‐dNLR评分(调整后的HR = 3.09, 95% CI = 1.01‐4.51)；校正后HR = 4.99, 95% CI = 2.32‐7.21(2分)与患者生存率降低显著相关，预测mGBC患者临床结局的AUC为0.756，明显高于单CEA和dNLR。

研究结果表明，预处理CEA‐dNLR评分在预测mGBC患者OS方面优于其他生物标志物，且它是该疾病的独立预后因素。
