本研究有3位阅片者参与来评价对所有有AD且最终诊断为BI RADS4或5病变的患者病变的显示情况(2D vs DBT)。利用可能评分量表评价病变的怀疑度。将2D钼靶检出和DBT检出的AD与病理结果相比较。利用McNemar和Pearson χ 2精确检验比较两种方法评价病变的发生率。

结果为,本研究共纳入有181处AD病变,其中2D钼靶检出122 (67.4%)处而DBT检出59 (32.6%)处。共哟42例女性患者(共有43处病变)在行DBT之前进行了2D钼靶检出。在共有121处AD病变的117例女性患者进行了2D钼靶和DBT,有59处病变(48.8%)仅在DBT检查中被发现。根据最终病理,在2D钼靶检出的病变(53 [43.4%] of 122)中,恶性肿瘤的发生率要明显高于DBT检出的病变DBT (6[10.2%] of 59) (P < .001)。超声检查联合2D钼靶检查(103 [84.4%] of 122)的频率要明显高于DBT (33 [55.9%] of 59) (P < .001)。对于DBT检查的AD,恶性肿瘤率在有(4[12.1%] of 33; 95% CI: 3.4%, 28.2%])无(2 [7.7%] of 26; 95% CI: 0.9%, 25.1%])超声检查参与时无显着性差异。基于影像科医生对病变的怀疑程度的NPV很好,但尚不足以免除活检。

本研究表明,尽管具有较高的活检符合率,与2D钼靶检出的可疑性AD相比,DBT检出可疑性AD的恶性率要低。
