GUT:丝氨酸蛋白酶可作为IBS中肠屏障功能障碍和症状严重程度指示物

设计:
本项研究共招募了39例IBS患者和25名健康志愿者,所有患者均完成问卷调查,并评估了肠道通透性,结肠屏障功能,紧密连接(TJ)蛋白功能,以及对粪便微生物群采用16s rRNA技术进行测序。同时基于酪蛋白的测定法测量了粪便上清液(FSN)中的丝氨酸蛋白水解活性(PA)。

结果:
研究结果显示IBS患者的粪便PA高于健康志愿者。8/20感染后IBS(PI-IBS)和3/19便秘为主的IBS的PA高(>95%)。高PA患者的排便更多,更不成形,症状严重程度更高,体内和体外结肠通透性更高。高PA FSNs增加细胞旁通透性,减少紧密连接蛋白的表达和增加磷酸化的肌球蛋白轻链(pMLC)表达。丝氨酸而非半胱氨酸蛋白酶抑制剂可显着阻断高PA FSN对屏障的作用。抑制蛋白酶激活受体2(PAR-2)可以减少对屏障的影响。与低PA患者相比,高PA IBS患者的紧密连接蛋白表达降低,微生物多样性降低。

结论:
IBS患者的粪便PA明显增高,症状更严重,肠屏障受损更厉害,微生物多样性降低。因此,丝氨酸蛋白酶活性可以很好的预测对IBS患者进行预测。
