镰状细胞病(SCD)与慢性凝血激活和静脉血栓栓塞(VTE)风险增加有关。镰状红细胞是SCD的主要病理特征,其血红蛋白异常聚合,导致红细胞形态发生巨大变化。

Camille Faes等研究人员采用SCD小鼠模型和SCD患者的血液样本来探究上述变化是否会影响镰状细胞血栓形成的结构、性质和动力学。

在镰状细胞小鼠体内形成的静脉血栓中发现RBCs发生镰状化、纤维蛋白沉积明显增多。

在体外凝块收缩过程中,从镰状体全血凝块中挤出的红细胞数量相比与从镰状体细胞特性和非镰状体凝块中释放的红细胞数量相比,在小鼠和人体内均显著减少。镰状红细胞的诱捕在很大程度上是XIIIa非依赖的,完全由镰状血的无血小板细胞部分介导。抑制磷脂酰丝氨酸,但不给药抗镰状体化合物,可增加了从镰状体血块释放的红细胞数量。

此外,SCD患者的全血(非血浆)血凝块对纤溶耐受,提示血液的细胞比例介导对t-PA的耐受性。镰状特性的全血凝块对t-PA既不耐受也不敏感。更换SCD患者的红细胞对全血凝块收缩正常化有长期的影响。而且,更换RBC可瞬时逆转全血镰状血凝块对纤溶的耐受性,部分是通过降低血小板来源的PAI-1。

总而言之,镰状血凝块的上述特性或可解释为何SCD患者发生VTE的风险增高。
