背景:在2型糖尿病患者中,尽管骨密度不变,但骨折风险增加。虽然这一明显的矛盾可能部分是因为胰岛素抵抗影响骨结构和/或物质属性,但很少有研究考察胰岛素抵抗和骨几何结构之间的相关性。

目的:在非糖尿病男性队列中探讨骨量峰值与年龄的相关性。

设计、研究地点和参与者:在大学研究中心采用基于人群的横断面研究996名25~45岁的非糖尿病男性。

主要结局测量:胰岛素抵抗使用胰岛素抵抗稳态模型评估(HOMA-IR)确定,该HOMA-IR从空腹血清样本中测量了胰岛素和葡萄糖。使用在桡骨远端、桡骨和胫骨干进行的外周定量计算机体层摄影术评估骨几何结构。

结果:年龄、身高和体重调整分析中,HOMA-IR与桡骨远端骨小梁面积和皮质骨面积、骨膜和骨内膜周长、桡骨和胫骨干极强应变指数负相关(β≤?0.13,P<0.001)。在调整了双能X线吸收法测量出的身体成分、骨转换标志物、肌肉大小或功能测量、脂肪细胞因子、瘦素、胰岛素样生长因子1、性类固醇水平后,这些相关性基本保持不变。

结论:在这项骨量峰值年龄的非糖尿病男性队列中,胰岛素抵抗与骨小梁和皮质骨大小负相关。在调整了身体成分、肌肉大小或功能、性类固醇水平后,该相关性仍然保持不变,表明胰岛素抵抗对骨几何结构的独立影响。
