Lancet Neurol:脑出血范围扩大**风险及影响因素预测模型

研究建立了一种脑出血患者出血体积增大的预测模型,该模型为脑出血患者预后预测及个体化治疗提供了方向

脑出血范围扩大与不良预后相关,近日研究人员就脑出血范围扩大的绝对风险因素及预测方法进行了考察,并评估了CT血管造影在脑出血预后评估中的应用价值。

本次研究为系统综述及荟萃分析研究,收集截止2015年12月31日的相关观察性及随机研究资料,入选研究至少包含10例急性脑出血患者,成年患者在症状出现后0.5-24小时内接受脑图分析,在6天后接受症状复查,患者基线脑出血体积小于150mL,未接受减少脑出血体积的相关治疗。研究定义复查时脑出血体积增加6mL以上为脑出血范围扩大,依据是否接抗凝治疗对患者进行分层。

36个研究,总计5435名患者纳入本次荟萃研究中,其中5076人在症状出现后第一时间未接受抗凝血治疗,这部分患者平均年龄67岁,1009人(20%)出现脑出血范围扩大。多因素模型显示,症状起始到基线显像时间(OR=0.50),脑出血基线体积(7.18),抗血小板药物(1.68),抗凝药(3.48)是独立的脑出血范围扩大预测因素,其C指数为0.78。在预测模型中增加CT血管造影斑点征(4.46)可增加模型的预测准确性(C指数增加0.05)。
