JCEM:饱和脂肪促进脂肪肝和神经酰胺升高

由此可见,饱和脂肪可显著诱导肝脏脂肪和血清神经酰胺,而膳食多不饱和脂肪可防止肝脏脂肪堆积,减少过量能量摄入时的神经酰胺和高脂血症水平以及超重个体的体重增加。

饱和脂肪(SFA)与多不饱和脂肪(PUFA)可能通过尚不清楚的机制促进非酒精性脂肪性肝病(NAFLD)。近日,内分泌和代谢性疾病领域权威杂志Journal of Clinical Endocrinology & Metabolism上发表了一篇研究文章,该研究旨在调查过度摄入富含SFA和PUFA的饮食是否导致超重和肥胖人群的肝脏脂肪积累不同。

该研究为双盲随机试验(LIPOGAIN-2)。过度摄入SFA与PUFA超过8周,然后进行4周的热量限制。该研究的参与者为超重或肥胖的男性和女性(n=61)。在习惯性饮食中添加高棕榈油(SFA)或向日葵油(PUFA)的松饼。 主要结局指标为瘦肉质量,次要和探索性结局指标包括肝脏和异位脂肪蓄积。 

体重增加情况在SFA组(2.31±1.38kg)和PUFA组(2.01±1.90kg)相似,P=0.50。SFA显著增加肝脏脂肪含量(相对增加50%)以及肝酶和致动脉粥样硬化血清脂质。相反,尽管体重增加相似,但PUFA不会增加肝脏脂肪或肝脏酶或对血脂造成任何不良影响。与PUFA相比,SFA对内脏脂肪、胰脂肪或全身脂肪的积累没有差异。PUFA减少循环神经酰胺水平,而SFA可使其持续增加。研究人员观察到可通过限制热量来逆转SFA的不良代谢作用。

由此可见,饱和脂肪可显著诱导肝脏脂肪和血清神经酰胺,而膳食多不饱和脂肪可防止肝脏脂肪堆积,减少过量能量摄入时的神经酰胺和高脂血症水平以及超重个体的体重增加。
