JAMA:肺气道发育水平影响老年人群COPD风险

在老年人中,气道发育不协调与COPD风险显著相关,相对于肺部,气道树径越小,COPD风险越大

吸烟是慢性阻塞性肺病(COPD)的主要危险因素,近日研究人员通过CT评估气道树径与肺部大小不匹配对老年人COPD发病和COPD肺功能下降的影响。
 
研究在美国和加拿大开展,分别有2531名和1272名参与者,通过CT对19个标准解剖位置的气道树径与肺部大小比值进行计算,主要结果为COPD,定义为支气管扩张后一秒用力呼气量与肺活量之比(FEV1:FVC)小于0.70,且伴有呼吸症状。次要结果为纵向肺功能
 
美国队列参与者平均年龄69岁,女性占52.7%,有237名(9.4%)COPD患者,平均气道与肺的比值为0.033,平均FEV1下降33mL/y。在2294名无COPD的患者中,98名(4.3%)发生COPD,中位时间为6.2年。与气道与肺比率最高的四分之一参与者相比,最低者的COPD发病率明显较高(每1000人年9.8例vs1.2例;比率比率[RR]:8.12),但FEV1下降无显著性差异(差异,2 mL/y)。在加拿大队列中,参与者平均年龄67岁,女性占44.3%, 113人(15.0%)患有慢性阻塞性肺病,中位数为3.1年,平均FEV1下降36mL/y。与气道与肺比率最高的四分之一参与者相比,最低者的COPD发病率明显较高 (每1000人年80.6 vs24.2;RR:3.33;),但FEV1下降也没有显著差异(差异1mL/y)。
 
研究认为,在老年人中,气道发育不协调与COPD风险显著相关,相对于肺部,气道树径越小,COPD风险越大。
