Radiology：超声评判的BI-RADS等级也可能不是那么准！

本研究旨在评价光声（OA）超声（US）在准确评价原BI-RADS为4a和4b的乳腺良性肿块降级为3或2的价值，并将结果发表在Radiology上。

本研究旨在评价光声（OA）超声（US）在准确评价原BI-RADS为4a和4b的乳腺良性肿块降级为3或2的价值，并将结果发表在Radiology上。

本研究旨在利用OA/US技术中激光来测量可疑乳腺肿块周围的含氧血红蛋白及脱氧血红蛋白的相对数量。本研究共纳入了209例患者，共有215处病变被US评为BI-RADS 4a或4b。根据临床病史及影像学表现，通过US对肿块进行首次评价，然后根据基于US信息恶性可能性和BI-RADS分类进行评价每一处肿块。随后利用OA/US对肿块进行再次评价。在OA/US评价过程中，放射科医生对5个OA/US特征进行评分，然后进行基于OA/US的POM和BI-RADS分类中。利用5个OA特征评分的加权数量评价OA/US BI-RADS降级或升级的百分比。

结果为，在OA/US中，共有47.9% (57/119； 95% CI： 0.39， 0.57) BI-RADS 4a和11.1% (3/27； 95% CI： 0.03， 0.28) BI-RADS 4b的乳腺肿块被降级为BI-RADS 3或2。2/7处原US为BI-RADS 4a恶性肿瘤被错误地降级，1/60处原US为BI-RADS 4b恶性肿瘤被错误地降级，整体假阴性为4.5% (3/67； 95% CI： 0.01， 0.13)。

本研究表明，通过OA/US，US评为BI-RADS 4a的乳腺肿块可被降级，这便能减少肿瘤的活检阴性率以及短期随访频率，而有少数可能会被错误地降级。
