J Endod：切开引流排脓对牙髓病患者的疗效评估：一项随机的预期单盲试验

目前为止，还未有预期的牙髓病学研究能够确定切开引流(I&D)对健康的牙髓病患者肿胀的治疗效果。这篇随机的预期单盲试验的目的是对诊断为牙髓坏死的有症状及肿胀的急诊患者实施牙髓清创术后，比较I&D术后放置引流条与模拟I&D程序及模拟放置引流条对疗效的差异。

目前为止，还未有预期的牙髓病学研究能够确定切开引流(I&D)对健康的牙髓病患者肿胀的治疗效果。这篇随机的预期单盲试验的目的是对诊断为牙髓坏死的有症状及肿胀的急诊患者实施牙髓清创术后，比较I&D术后放置引流条与模拟I&D程序及模拟放置引流条对疗效的差异。研究对81名临床检查发现脓肿的成年急诊患者给予青霉素（如果过敏给予克林霉素）治疗，并实施了牙髓清创术。随后，随机分为2个治疗组：I&D术后放置引流条和模拟I&D程序及放置引流条。在治疗结束时，对所有患者给予布洛芬/对乙酰氨基酚联合用药，如果需要，给予阿片类止疼药。记录患者术后4天的疼痛和药物使用情况。治疗成功的定义为术后没有或轻微的疼痛以及未使用阿片类止疼药。采用混合模型逻辑回归进行反复检测评估治疗成功与否。结果显示，2组在术后4天的疼痛及药物使用均减少。模拟I&D组的治疗成功率要明显高于I&D组(odds ratio = 2.00； 95%置信区间， 1.16-3.41)。模拟I&D组的治疗成功率为45%，I&D组为33%。结论：在牙髓清创术后，接受了模拟I&
