Eur Respir J:支气管哮喘患者血嗜酸性粒细胞与肺功能下降的关系!

由此可见,血嗜酸性粒细胞与气流阻塞和肺功能的加速下降有关,独立于哮喘和吸烟。嗜酸性粒细胞增多症是气流阻塞的危险因素,即使在没有症状的人群中也是如此。

嗜酸性粒细胞炎症和气道重塑是哮喘的特征,但两者之间的相关性尚不清楚。近日,呼吸疾病领域权威杂志Eur Respir J上发表了一篇研究文章,研究人员评估了年轻人群中血嗜酸性粒细胞与肺功能下降之间的关联。

在调整性别、吸烟、哮喘和18岁时肺活量后,研究人员使用线性混合模型分析血液嗜酸性粒细胞与21、26、32和38岁时测定的肺活量之间的相关性。研究人员进一步分析平均嗜酸性粒细胞计数与从21岁到38岁之间肺活量变化之间的相关性。

研究人员发现较高的嗜酸性粒细胞与支气管扩张前、后较低的1秒用力呼气量(FEV1)/用力肺活量(FVC)比值以及较低的FEV1%预测值相关(所有P值均小于0.048)。虽然哮喘患者嗜酸性粒细胞计数较高,嗜酸性粒细胞与肺活量之间的相关性在没有哮喘的参与者中是相似的。21~38岁的平均嗜酸性粒细胞计数>0.4×109个细胞L·L-1的受试者FEV1/FVC比值(差异为1.8%、95%CI为0.7~2.9%、P=0.001)和FEV1值(差异为3.4%、95%CI为1.5-5.4%)均显著低于低计数组。

由此可见,血嗜酸性粒细胞与气流阻塞和肺功能的加速下降有关,独立于哮喘和吸烟。嗜酸性粒细胞增多症是气流阻塞的危险因素,即使在没有症状的人群中也是如此。
