为了探讨心肌纤维化(MF)在有HFpEF和有HFpEF危险的患者中是否同样普遍,与疾病严重性和结局是否有相似的联系,近日在JAMA上发表的一篇文章进行了一项观察性队列研究,该研究从2010年6月1日至2015年9月17日,随访至2015年12月14日,在一个心血管磁共振(CMR)中心进行。有持续收缩功能的CMR患者符合条件。心血管磁共振用来排除心机淀粉样变的患者(n=19)。通过细胞外容积(ECV)CMR测量定量心肌纤维化。主要的结局是基线脑钠肽(BNP);随后因心力衰竭住院或死亡。

该研究共纳入1174例患者(537例[46%]女性;平均[间距范围{IQR}年龄,56[44-66]岁),250例因为升高的BNP水平而有HFpEF"风险";160例有临床诊断性HFpEF的记录;745例没有HFpEF。无论是有HFpEF风险或有HFpEF的患者与没有HFpEF的患者相比,表现出相似的较高的MF患病率/程度和较差的预后。在有HFpEF风险或有HFpEF的患者中,HFpEF的实际诊断与在MF(中位数ECV,28.2%;IQR,26.2% - 30.7%vs28.3%;IQR,25.5% - 31.4%;P =0.60)或预后(等级0.8;P =0.38)的显着差异并不相关。经过平均超过1.9年的随访,61例有HFpEF风险或有HFpEF的患者发生了不良事件(19例因为心力衰竭住院;48例死亡;6例两者都有)。在有HFpEF的患者中,在多变量线性回归模型中ECV与基线BNP相关;在多变量Cox回归模型中与结局相关(eg,ECV每增加5%,危险比增加1.75,95%CI,1.24-2.45;P=0.001)而无论分组患者有HFpEF风险或无。

由上述研究可见在HFpEF发展过程中发生的万千变化中,BNP升高是普遍的,而MF在这些有HFpEF风险或有HFpEF的患者中也普遍。由此可推论,MF可能先于HFpEF诊断。无论如何,MF与疾病严重性(即BNP)和结局有关。而介导MF的细胞是否能成为HFpEF治疗的目标有待进一步研究。
