近日,神经病学领域权威取杂志Neurology上发表了一篇研究文章,研究人员旨在明确镰状细胞病患者(SCD)对梗死的区域脆弱性机制,通过测量SCD儿童脑血流(CBF)、氧摄取分数(OEF)和脑氧利用代谢率(CMRO2)。

受试者进行了大脑核磁共振成像检查,以测量CBF、CMRO2和OEF。梗死热图是由一个独立的儿童SCD队列来创建,并与前瞻性获得的OEF地图相比较。

在五十六名受试者中,有36名SCD儿童和20名对照儿童完成了研究评价。SCD队列全CBF(99.2 vs. 66.3毫升/ 100克/分钟,P<0.001),OEF(42.7% vs. 28.8%,P<0.001)和CMRO2(3.7 vs 2.5毫升/ 100克/分钟,P<0.001)均高于对照组。SCD队列区域OEF峰值在深部白质中确定,通过SCD与对照者比较平均OEF的比率图来描述。这一区域的CMRO2,包含了CBF的最低点,较所有白质相对较低(P<0.001)。此外,来自于一个独立的SCD队列,该区域OEF峰值位于最大梗死密度区域。

由此可见,在深部白质增加的OEF可以确定SCD儿童增加卒中风险的代谢应激性脑组织。研究人员建议扩展生理区域的边界,慢性贫血缺氧会加剧这一区域,也就解释了这个区域的卒中高风险。
