近日，心血管领域权威杂志Eur Heart J上发表了一篇研究文章，研究人员进行了一项系统评价和荟萃分析，以调查三尖瓣关闭不全（TR）严重程度对死亡率的影响。

研究人员对报告TR患者临床结局的研究进行了系统检索。主要终点为全因死亡率，次要终点为心脏病死亡率和因心力衰竭（HF）住院治疗。研究人员通过荟萃分析对符合条件的研究进行效果估计，根据TR的严重程度得出每个终点的总风险比（RR）和95％置信区间（CI）。

该分析共纳入了70项研究和32601名患者，平均（±SD）随访3.2±2.1年。与无/轻度TR患者相比，中度/重度TR患者死亡风险增加2倍（RR为1.95,95％CI为1.75-2.17）。在13项调整了收缩期肺动脉压（RR为1.85,95％CI为1.44-2.39）和15项调整了右心室（RV）功能障碍（RR为1.78，95％CI为1.49-2.13）的研究中，中度/重度TR仍然与全因死亡率相关。中度/重度TR也与心脏病死亡率增加（RR为2.56,95％CI为1.84-3.55）和HF住院治疗（RR为1.73,95％CI为1.14-2.62）有关。与无TR患者相比，轻度、中度和重度TR患者全因死亡风险逐渐增加（RR分别为1.25、1.61和3.44;趋势P<0.001）。

由此可见，中度/重度TR与死亡风险增加相关，其似乎与肺压和RV功能障碍无关。
