Sci Rep:结直肠癌免疫治疗新发现,CD20+B淋巴细胞有效改善预后!

近日,一篇发表在Scientific reports的研究发现:肿瘤浸润性CD20+B淋巴细胞在结直肠癌(CRC)患者预后中展现出积极作用。此外,CD20+B淋巴细胞和细胞毒性T淋巴细胞在CRC患者的预后具有协同作用,这为结肠癌的预后带来了重要线索,也有助于挖掘新的治疗靶点。



适应性免疫反应是由抗原特异性T和B淋巴细胞共同协调的。肿瘤免疫以T细胞免疫为主,肿瘤浸润淋巴细胞(tumor infiltrating lymphocytes,TIL)属于T细胞中的一种,在肿瘤组织中TIL浸润被视为机体对肿瘤的一种局部免疫反应,通过检测直肠肿瘤活检样本中TIL的密度可以预测新辅助化疗的疗效及敏感性。浸润性T淋巴细胞亚群在结直肠癌中的预后重要性已被广泛认可,甚至医学界于将基于免疫组化(IHC)评估T细胞标志物的免疫评分引入临床实践。然而,浸润性B淋巴细胞的作用尚不清楚,在预后方面的价值也有待探索。

在本研究中,使用多路复用IHC(多路复用IHC和多光谱成像已被证实为研究肿瘤免疫渗透的一种有效方法)和多光谱成像,研究人员分析了大肠癌组织标本中五种不同的免疫细胞(CD20+B淋巴细胞、CD8+细胞毒性T淋巴细胞和FOXP3+T调节细胞)和先天性(CD68+)的浸润程度巨噬细胞和CD66b+中性粒细胞)免疫系统。由此可以探究B淋巴细胞的个体临床相关性和预后重要性,以及与其他免疫细胞亚群的相互关系和它们的联合预后价值。

在这项研究中,研究人员观察到肿瘤浸润性CD20+B淋巴细胞数量较高的患者预后良好,这与Berntsson等人先前的研究一致,他们在一个相对较大的队列中发现浸润性CD20+B淋巴细胞在CRC原发肿瘤中具有独立的有利预后作用。在之前的研究中,研究小组使用了一种评分方法,手动计数肿瘤浸润CD20+细胞,并将其分成高、低肿瘤浸润组来分析其预后价值,结果发现CD138+浆细胞的浸润与预后改善有关。然而,他们并没有对B和T淋巴细胞浸润进行联合分析。

因此,在本次研究中,该研究团队着重于CD20+B淋巴细胞在结直肠癌中的预后重要性及其与其他免疫细胞亚群的关系。他们证明了CD20+B淋巴细胞高度浸润的肿瘤患者具有独立的预后优势。而且,CD20+B淋巴细胞的浸润与其他免疫细胞亚群的浸润高度相关,而CD20+B淋巴细胞的预后作用可能仅仅部分是通过与细胞毒性T淋巴细胞的协同作用而介导的。

综上,本次研究结果支持肿瘤浸润性CD20+B淋巴细胞在结直肠癌患者预后中的积极作用。此外,CD20+B淋巴细胞和细胞毒性T淋巴细胞的协同预后效应也被认为是有效的。本次研究再次推动了CRC领域免疫治疗的进展,可以预见,在不久的将来,CRC患者的治疗格局将因此而发生翻天覆地的变化,免疫治疗极有可能会成为主流治疗手段。
