Obstet Gynecol：HIV女性使用左炔诺孕酮类宫内节育器与含铜节育器的比较

节育器是一种放置在子宫腔内的避孕装置，其种类很多。不带药的节育器称惰性宫内节育器，如宫内节育器加上孕激素或铜，可提高避孕效果，称之为带药或活性宫内节育器，是目前推崇的节育器械种类。研究者在乌干达梅勒高医院进行了一项双盲随机对照试验，探究HIV女性对于带左炔诺孕酮或铜的节育器的停用率。研究的初始结局指标为IUD置入1年内的宫内节育中止率，继发结局指标为严重副反应或不良事件的发生率。从2013年9月到

研究者在乌干达梅勒高医院进行了一项双盲随机对照试验，探究HIV女性对于带左炔诺孕酮或铜的节育器的停用率。研究的初始结局指标为IUD置入1年内的宫内节育中止率，继发结局指标为严重副反应或不良事件的发生率。

从2013年9月到2014年12月间，研究者从979名候选人中筛选出703进入该研究，其中349名为铜组，354名为左炔诺孕酮组。随访期间，IUD置入1年内，宫内节育中止率在铜组为8.6% (29/338) ，该数据在左炔诺孕酮组为8.1% (27/334) (发病率 1.1 [95% CI 0.64-1.96])。

严重出血事件更多见于铜组：37% （125/338），该数据在左炔诺孕酮组为19.5%（65/334）。不过闭经时间更易发生在左炔诺孕酮组：19.8% (66/334) ，该数据在铜组为3.3% (11/338) 。

研究结果表明，宫内节育器不管是带有左炔诺孕酮还是铜，其使用过程中的停用率没有统计学差异。左炔诺孕酮类节育器有更低的出血风险和更高的闭经发生率。
