Nature Materials:癌症液体活检获重大突破

这一研究成果公布在10月9日的Nature Materials(影响因子39.737)杂志上,文章的通讯作者分别为中山大学肿瘤防治中心徐瑞华教授(同时也为第一作者),以及加州大学圣地亚哥分校张康教授(Kang Zhang)。

首次报道ctDNA早期诊断癌症

肝细胞癌(Hepatocellular carcinoma ,HCC)是一种成人最常见的原发性肝癌,也是造成全球癌症死亡的主要原因之一,每年HCC新增病例多达78万例,死亡数量也高达74万例。这种恶化程度较高的致死疾病与肝部慢性炎症相关,目前没有特效治疗手段。而且HCC早期比较隐匿,患者确诊的时候往往已经到了中晚期,这个时候进行治疗效果当然不会很理想。因此一直以来,科学家们都希望能在早期发现HCC。

“HCC及其前体,非酒精性脂肪性肝炎,在过去的十年中影响了很多人。这项最新成果对于治疗和预防全球范围内这种毁灭性疾病具有重要意义,同时它也是第一次报道利用ctDNA早期诊断癌症的可能性,”Moores癌症中心主任Scott Lippman说。

目前HCC的检测方法主要依靠影像学和非特异性肿瘤标志物甲胎蛋白(AFP)的血液检测,而AFP通常只有在疾病发生到中晚期的时候才会升高。

“非侵入式血液检测或液体活检是一个更好的选择,”张康教授表示,“但是,在研发HCC筛选有效血液检测方法上一直没有显著的成果,目前唯一的血液检测就是AFP,这种方法灵敏度低,限制了临床应用。”

ctDNA的甲基化水平

许多液体活检都是通过循环肿瘤DNA(ctDNA)进行检测,ctDNA就是肿瘤细胞遗传物质脱落后进入血液的碎片。这种活检方式比其他的癌症检测方法具有潜在的优势,因为首先它是微创的,可以在治疗期间随时进行,从而医生可以实时监测肿瘤的分子变化。而且这不会因为成像的不确定性而发生误判,最后ctDNA也可能代表了患者整体的肿瘤状况,而不仅仅是局限于受检测部分的肿瘤。

DNA甲基化是一个调控基因表达的过程,在这个过程中DNA是否甲基化决定了基因是否能表达。抑癌基因的甲基化水平增加是肿瘤发生的早期事件,这表明DNA甲基化模式的改变可能是肿瘤发生的一个很好的指标。但是ctDNA在血液中的含量极微,每毫升血中仅有约20ng,相当于一滴水的一亿分之一,并且混杂在更大量的正常游离DNA背景中,在这么微量的ctDNA中,还要检测单个碱基的甲基化水平,非常困难。

在最新这项研究中,研究人员先后攻克了稳定提取微量ctDNA、提高重亚硫酸盐转化效率、靶向甲基化PCR扩增及测序、海量数据的统计学分析处理等一个个技术壁垒,终于从40多万个候选位点中分别寻找到10个早期诊断和疗效相关以及8个预后相关的位点。

研究人员发现这10个早期诊断位点的甲基化水平在总共1098例肝癌患者和835例健康人的研究人群中显示出高达84.8%的诊断敏感性和93.1%的特异性,还能准确的预测肿瘤的分期、疗效和复发。

这意味着利用这一方法,肝癌患者的漏诊率将比AFP降低一半以上,早期肝癌患者及时确诊以后,将更有机会接受根治性治疗,其预后将得到极大的改善。而利用8个预后相关位点的甲基化水平则能准确的预测不同患者的生存和预后,有利于指导医生对不同的患者进行更为个体化的精准治疗,例如对预后良好的患者避免给予过度治疗,而对复发高危患者则给予更为积极的辅助治疗等。

“我们研究的结果非常令人鼓舞,”张康教授说,“在一个大型的临床实验中,我们基于血液的HCC诊断与肿瘤负担、治疗反应和癌症阶段密切相关。目前肿瘤学家受限于如何检测HCC和评估治疗疗效,而我们的成果提出了一种新的、更有效的方法,可以适用于实体恶性肿瘤,比如HCC等其他疾病。”
