气道阻力的增加与低氧或高碳酸血症相关,导致频繁的睡眠觉醒,尽管其通常会终止睡眠呼吸暂停的发作,但也会导致异常的睡眠结构,导致轻睡眠,正常睡眠减少。也有研究报道显示OSA患者生活质量及可靠的人际关系受损,交通事故发生率增加,工作和学习效率下降。然而,最广泛被识别到的OSA症状为白天睡眠增多。

OSA最有效的治疗方法之一为持续正压通气治疗(CPAP)。CPAP是由一个气压产生装置和一个封闭的适配口罩组成,可维持上呼吸道开放,防止与片段睡眠相关的气道梗阻。目前英国NICE指南推荐CPAP作为中重度OSA和睡眠过多患者的一项成本效益较好的治疗方式。然而,并不是所有的OSA患者均存在白天嗜睡的症状,在有些患者中,神经缺损症状可能是主要的中枢神经系统表现。

两项近期的研究(MOSAIC和PREDICT)表明CPAP的治疗获益可分别扩展至症状最轻微和年龄较大的OSA患者中。理解OSA治疗的疗效和成本获益越来越重要,由于导致其发病的最显著的两种危险因素肥胖和老龄化的加重,OSA在未来可能会成为高发病率的一种严重的健康问题。

阻塞性睡眠呼吸暂停的病理生理学

在阻塞性呼吸暂停期间,会出现脑血流量的改变以及呼吸暂停导致的低氧血症,脑灌注的减少可能使患者处于夜间脑缺血状态之中。此外,研究还显示OSA患者觉醒状态的低灌注也会改变某些脑区静息态脑血流模式。

大量的临床研究显示OSA患者脑电图会发生改变,包括异常的皮质兴奋性以及与之相关的认知功能障碍。这些研究共同描绘了一个假定的神经环路受损,包括OSA诱导的脑损伤以及小脑-丘脑皮层环路功能紊乱,并累及海马形成。

既往研究已经显示OSA患者中会出现一系列常见的症状,包括抑郁,注意力障碍,思维和感觉障碍,执行功能和言语记忆障碍;这与其他两种神经系统临床综合征的症状类似,即丘脑皮层节律障碍和小脑认知情感综合征。相对应地,临床和动物研究显示OSA最常受累的解剖部位为小脑神经环路的调节以及丘脑(基底节)与前额叶皮层之间正常血流形成(图1)。

此外,研究报道一些其他的神经精神疾病也常与OSA合并发生,或与OSA密切相关。例如,癫痫的成人患者OSA的风险似乎增加。类似地,OSA与年龄较大的成人癫痫患者抽搐症状加重相关,而CPAP治疗可能是在这些人群中改善抽搐控制的一种重要方法。睡眠呼吸暂停也被认为是卒中的一种重要的危险因素。其被认为会加重卒中过程中的神经损伤,增加再发卒中的风险。

再者,越来越多来自动物研究的证据显示间断性低氧会促使脑淀粉样蛋白形成和tau蛋白磷酸化,而这是阿尔茨海默病(AD)的最主要两大特征。在夜间缺氧发作期出现的间歇性低氧和活性氧自由基会促使动物皮层和脑干神经元发生退行性变以及轴索功能障碍。中枢神经系统的少突胶质细胞以及髓鞘生成细胞也对低氧和片段睡眠具有选择性的高敏感性。

然而,尚不清楚这种特殊的易感性在多大程度上导致了某些OSA患者脑内广泛存在的萎缩性性的脑白质改变(比如穹窿和胼胝体)。然而,与临床前研究结果一致,一些临床研究也显示了一些年龄较大的OSA患者出现脑萎缩,认知功能障碍,痴呆的发生和严重程度的加重。而相反,在OSA儿童患者中显示出现学习能力下降,多动性增加以及注意力缺陷多动症的发生率增加。

在生命阶段中独特的易感性时期,以及遗传,环境和生活方式等因素似乎决定着间断低氧对于中枢神经系统的影响。其中有一些影响出现在生命的前后两端的情况也可能反映了间断缺氧在神经发育髓鞘形成过程中直接的和后天的影响。这种过程在人类中,不像在其他哺乳类动物中,会跨越几十年,并且可能导致晚年的认知功能。

尽管存在上述的这些改变,但OSA相关的脑损伤通常是较轻微的,并且与之相关的神经认知功能损害也常常是轻度且弥漫性的,其是否可完全性或部分性被CPAP治疗所逆转是存在争议的。这种不一致也导致长久以来在不同研究中使用不同的影像学分析方法,不同的统计学阈值以及缺乏OSA特异性的敏感性神经认知功能检测量表。

此外,在OSA期间对一个给定的低氧刺激不同个体之间的异质性以及睡眠对局灶性神经易感性的影响也被认为是这种异质性的来源。夜间低氧-复氧循环所致的缺血预适应带来的心血管和脑血管保护作用也被认为发挥了重要的作用。

阻塞性睡眠呼吸暂停与缺血预适应

缺血预适应代表着多种细胞对缺血的广泛适应过程。阻塞性睡眠呼吸暂停综合征被认为是预适应及对间断性缺氧适应性反应过程形成的一个临床例证。在OSA中,缺血预适应被认为是由于一些基因程序的激活所致,包括低氧诱导因子-1,血管内皮生长因子,促红细胞生成素,心房钠尿肽以及脑源性神经生长因子等。

多年以来,已显示有不同的内在机制以及通路发挥了作用,包括膈神经对运动输出的长期易化作用,化学反射的激活,血管重述,血管生成,自噬,反应性胶质细胞增多,不同的突触改变等,近期研究还显示神经发生的调节也有作用。慢性间歇性缺氧和片段睡眠是OSA诱导的CNS损伤的最主要的两大特征,这些都会减少啮齿类动物中海马的神经发生。然而,低氧和缺血损伤也被认为是成人神经发生的刺激因子。

Gozal等研究表明间断性缺氧暴露后期会出现海马齿状回增殖改善,尽管在损伤持续发生的情况下也会出现;随后的研究表明齿状回增殖短暂性改变会引起后期临床功能的部分恢复。因此,其他临床前研究已经证明了适度间歇性缺氧的保护性,这进一步支持了缺血预适应样过程的概念。

例如,在啮齿类动物模型中,在缺血性事件之前进行间歇性缺氧干预可增加脑源性神经营养因子的表达,增加神经发生和功能性突触发生,改善空间学习和长期记忆障碍。在另一项研究中,成年大鼠间歇性低氧会促进海马神经发生,可类似于抗抑郁样疗效。然而,需要注意这些研究也提示间歇性缺氧暴露的持续时间,缺氧的强度,以及相关的氧化应激是决定间歇性缺氧是保护性的还是有害的重要决定因素。

海马结构被认为是OSA受累的神经环路的“边缘叶”的核心部分。海马与大脑的其他区域,包括丘脑,小脑等在内都有相互作用,通过两个主要的系统进行作用,即内嗅皮层和穹窿伞部通路。海马各个部分功能各不相同:空间处理可能在背侧海马,而焦虑相关行为依赖于腹侧海马区(图2)。

海马在学习和记忆功能方面是十分关键的,其与前额叶和顶叶区域之间的连接调节一系列注意力、记忆和情感过程。OSA的CPAP治疗可部分性逆转该区域的损害,减轻一些相关的功能损害,也有可能是通过调节海马神经发生所致。然而,该研究的结果尚未被重复,需要进一步研究。

研究已显示在小脑顶核固有神经元激活后会出现条件性的中枢神经源性神经保护作用。对这些核团的神经刺激似乎能够提供“保护性”的作用,减少接下来的缺血发作过程中皮层神经元的兴奋性,减少脑部微血管的免疫反应。

研究指出在一些较年轻的轻度OSA患者中由于海马肥大而导致“代偿性”小脑的受累。尽管在海马和小脑之间没有直接的单突触解剖连接,但两者之间的连接性被认为对于情绪加剧和新情境下的运动控制和联想学习功能十分重要。

海马通过脑桥,网状核和橄榄核与小脑相连接,而下行环路是通过顶核和丘脑连接。最近,研究显示海马θ振荡在协调广泛分布的联想学习记忆系统中发挥了重要作用,其中小脑也是该系统中的一部分(图2)。此外,一些研究显示,海马θ振荡在海马的神经发生的过程中也发挥了作用,能够调节海马的功能性成分(图2)。

在近期的一项神经影像学研究中显示,OSA患者中会出现萎缩性 (如胼胝体)和肥大性(如海马体积和小脑功能性连接)改变同时存在。这些改变或许反映了OSA相关脑损伤的发展本质以及脑内内源性修复系统发挥功能过程中不同的毒性事件和神经炎性过程的内在的相互作用,比如缺血预适应和脑可塑性增加。

睡眠呼吸暂停与睡眠结构的紊乱

频繁的部分性睡眠觉醒,以及间歇性低氧和高碳酸血症是OSA的核心特征。它们共同导致异常的睡眠结构,更轻更少的恢复性睡眠以及过多的白天嗜睡的症状。就这种紊乱睡眠的真正后果,领域内的专家意见不一。新型的具有某些争议性的突触体内稳态假说(SHY)提出,睡眠是大脑为可塑性所支付的价格。

睡眠,及其慢波的成分,REM激活和睡眠纺锤波的作用可通过重整网络突触强度,恢复细胞内稳态来维持大脑可塑性,这是目前转化神经科学领域我们了解最少的一个领域之一。相应地,我们对于OSA诱导的脑损伤的神经机理也知之甚少,例如重复的觉醒会干扰不同时期的睡眠。

已有研究发现,OSA患者中,N2期非快速眼动(nREM)睡眠增加,而N1,N3期和REM期睡眠减少。此外,近期的一些临床研究表明,OSA患者夜间睡眠中睡眠纺锤波的时间和空间演变过程是紊乱的。

多种形式的动物研究表明片段睡眠独立影响中枢神经系统脑区,与间歇性低氧受影响的脑区相类似。睡眠剥夺对普通人群认知影响的临床研究表明出现与OSA患者中类似的损害。传统上报道的对认知系统最严重的影响依赖于情绪的数据。在OSA患者中,睡眠中会出现对各种神经递质(如去甲肾上腺素)的调节,这可能会改变海马杏仁核环路的学习功能。

此外,也许与SHY和其他睡眠和记忆理论相一致,一些临床和社区人群研究表明在nREM睡眠期间发生的阻塞性事件与更严重的认知功能缺损或者生活质量受损相关。另一方面,在OSA患者中REM时间似乎与更多的交感神经活性和心血管不稳定性相关。

结论

上述文字简单地总结了目前有关OSA独特类型以及神经病理分布之间假定的相关性,及其相关的功能性改变。清楚的是,OSA诱导的脑损伤取决与患者个体临床情况以及病理过程的阶段。因此,迄今为止,有关OSA功能性“神经连接蓝图”的独特性尚未完全清楚。未来的临床研究应该利用多种方法,使我们更加了解病理过程,以及治疗之后的可逆性。

很显然,对OSA的中枢神经系统后遗症的早期发现是至关重要的,因为这样可以使其在所有症状都完全出现之前就给予及时的治疗。在机制方面,有关不同的睡眠时期与OSA严重程度之间相关作用的进一步研究将会增强我们对于老年OSA患者认知功能下降的理解和认识。
