生长分化因子-15(GDF-15)是转化生长因子(TGF)-β细胞因子超家族的一个成员,其在组织中广泛表达,可能对组织损伤有反应而被诱导。GDF-15水平的升高可能是参与CKD患者肾功能损伤的新途径。在临床表型和资源库(C-PROBE)研究和西雅图肾脏研究(SKS)的参与者中,研究人员检测了肾脏组织中的GDF15 mRNA表达是否与循环中GDF-15水平相关,并且循环中GDF-15水平的升高是否与肾功能下降有关。

在配对样本的来自于C-PROBE研究的24例CKD患者中,循环中GDF-15水平与肾脏组织内GDF15转录水平显著相关(r=0.54,P=0.01)。在224例C-PROBE研究与297例SKS研究的受试者中,分别有72例(32.1%)和94例(32%)达到了复合终点,在中位数为1.8年和2年的随访中分别有eGFR下降30%或进展到终末期肾病。

在多变量模型中,调整了潜在的混杂因素后,GDF-15水平每增加一倍在C-PROBE研究和SKS研究肾脏疾病发生进展的风险分别增加了72%(95%可信区间为1.21-4.45;P=0.003)和65%(95%可信区间为1.08-2.50;P=0.02)。
