囊性纤维化:抗生素相关性肾损伤的检测

遗传性疾病囊性纤维化(CF)的特征是继发性细菌性肺部感染,常由特定的耐药细菌铜绿假单胞菌所致。而氨基糖苷类抗生素对这种细菌具有良好的作用,常被用于治疗这些感染。但是,氨基糖苷类抗生素可能会损害肾脏。尽管有降低风险的策略,但氨基糖苷类抗生素的暴露与囊性纤维化患儿的急性肾损伤(AKI)密切相关。

目前用于评估肾脏损伤的方法依赖于血清肌酸酐的测量,血清肌酸酐是肾脏过滤能力的量度。不幸的是,这种方法只会在重大损害已经发生时显示出差异,因此血清肌酸酐检测指标对儿童来说具有明显的缺陷。为了鉴定肾损害风险的增加,则需要开发改进的生物标志物,其不仅应反映毒性部位,而且可以在更早的阶段识别损伤。

为了寻找这种改进的生物标志物,来自利物浦大学和伦敦大学学院的研究人员招募了超过15020岁以下的确诊为CF儿童和青少年。研究人员发现患者尿液中KIM-1NGAL的浓度在暴露于氨基糖苷类抗生素时有所增加,这些增加发生在血清肌酐不升高的情况下,因此可能代表患者早期的肾功能损伤。KIM-1的基线(治疗前)浓度随着氨基糖苷类抗生素的累积而增加,这也表明KIM-1可能是识别慢性肾损伤的关键生物标志物。
