JAMA:健康的生活方式可降低痴呆风险,遗传高风险人群也可以受益

研究认为,不良的生活方式和较高的遗传风险都与更高的痴呆风险显著相关。在遗传风险高的参与者中,良好的生活方式可降低痴呆风险

遗传因素会增加痴呆症的风险,但生活方式因素对痴呆的影响尚不清楚。近日研究人员就健康的生活方式对痴呆风险的影响进行了考察。

60岁及以上欧洲成年人参与,在基线时无认知障碍或痴呆,随访至2016年或2017年。考察具有低(最低五分之一)、中等(2至4)和高(最高五分之一)风险类别的痴呆的多基因风险评分和加权健康生活方式评分,包括不吸烟、定期体育活动、健康饮食和适度饮酒,分为有利的、中等的和不利的生活方式。

共有196 383人(平均年龄64.1岁;52.7%为女性),随访1 545 433人年。总体上,68.1%的参与者遵循良好的生活方式,23.6%的人遵循中等的生活方式,8.2%的人遵循不良的生活方式。20%的患者具有较高的多基因风险评分,60%的患者具有中等风险评分,20%的患者具有较低的风险评分。在遗传风险较高的人群中,痴呆发生率为1.23%,而低遗传风险者为0.63% (调整后的危险比为1.91)。在遗传风险高、生活方式不良的人群中,1.78%参与者发生痴呆,而低遗传风险和良好生活方式的痴呆发生率为0.56% (HR:2.83)。遗传风险与生活方式因素之间无显著交互作用(P=0.99)。在高遗传风险人群中,生活方式良好者痴呆发生率为1.13%,生活方式不良者为1.78% (HR:0.68)。

研究认为,不良的生活方式和较高的遗传风险都与更高的痴呆风险显著相关。在遗传风险高的参与者中,良好的生活方式可降低痴呆风险。
