90%的血氧饱和度,患者活蹦乱跳,这是咋回事?

“兄弟,你帮我来做个评估,恩,呼吸科已经有人来会诊过了,但是我觉得没有说服我,你再来帮我评估一下。”

消化科的沈医生是我们同届的同学,工作超过15年的医生现在都是各个专科的主力军了,常规会诊主管医生不会指定谁来看,除非他觉得病情让他一头雾水,其他人会诊也没有解决问题的时候,才会打到“弟兄牌”。

“OK,说一下状况。”我一口答应他,我知道这兄弟的内科功力不弱,他觉得纠结的状况,也不会太简单。

“77岁的老太太,胆道结石,需要做ERCP手术,但是有明显的低氧血症,从其他医院转诊过来。你帮我评估这个低氧血症能不能做ERCP手术”。

“血氧水平会随着年龄下降,你都还给生理老师啦?!”我笑话他。看血气分析,可是内科医生的基本功夫。

“问题是,我觉得这病人很奇怪,就是奇怪,所以叫你来看,来给我好好看看。”高年资的医生蛮起来,也是很蛮,我在电话那头笑。

“感觉奇怪”是一个高年资医生很特别的直觉,那是不能随意放过的。于是我去消化科会诊他的“奇怪”病人。

邱阿姨(化名)坐在床前和女儿聊天,如果不说,我根本看不出她就是那个“低氧血症”的病人。

“我没有什么不好,心脏没有病,没有气管炎,还可以爬楼。”她连续而清楚地回答我。

的确不像一个呼吸功能有大问题的病人。但是氧饱和度夹子夹在她手上的时候,显示的数字是90!一个缺氧到接近危险的数字。

我心里嘀咕了一下,把夹子夹到自己手上试了试,这种氧饱和度夹子有时候会出点小状况。99!我的氧饱和度很好,明显,今天是错怪了这个夹子,它没有出状况。机器的检测结果是可信的。

没有气管炎;没有心脏病;没有呼吸困难;没有端坐呼吸;没有紫绀;没有浮肿;没有心脏杂音、没有肝脏肿大、没有不舒服......

我问了一遍病史、查了一遍体检,好像什么问题也没有看出来。-—难怪沈医生要奇怪了,低氧是的确有的,氧饱和度夹子显示的数值一直没有超过90。

看我又调整了一下夹子的位置,陪我一起看病人的沈医生摊摊手,“是不是很奇怪?

我点点头,承认他“奇怪”的有道理。

呼吸衰竭低氧血症的五大原因分别是:吸入气氧浓度不足、通气障碍、弥散障碍、通气血流比失调和动静脉系统间分流。

现在,前面3种已经给病史、查体排除得差不多了。

“拿储氧面罩来”。我对床边的护士说。单纯的通气血流比例失调可以被吸入纯氧纠正,而分流不行。

我加大了氧流量,同时使用了储氧面罩,使得吸入氧浓度接近100%。

“我很好,医生你们在试验什么?”邱阿姨带上面罩,有点莫名其妙地问我。

氧饱和度夹子的数值往上窜升了一点,但却没有完全达到预想中的100%,是而停在95%左右徘徊。

“分流?”我看看沈医生。他马上送了他那个精致强悍的进口听诊器到我眼前。

“心脏没有杂音,我已经听过了,超声也做过了,心腔内并没有缺口。”

我没有接他的听诊器,刚才已经听得够清楚,的确是没有心脏杂音,况且,超声的结果也明明白白。

举起病人的术前X线平片,对着床外的光线细看,我发现一个不太明显的阴影,左侧心影遮挡的后方脊柱旁出现一个不起眼的黑色阴影!



“ok,可以了。”我对邱阿姨笑一笑,转身离开病房。

“可以了?”沈医生莫名其妙地跟出病房。会诊嘎然而止,结束得太快太突然,他看着我问:“就这样?”

“做一个肺血管的增强CT,下午就知道结果了。”我故意不告诉他结果,顿了一顿,说:“没事,她可以做ERCP,你只管放心做。”

“如你所料,确实是肺部的动静脉分流。”下午,沈医生的电话来了,悻悻的语气,言若有憾心则喜之。



CT上,在患者左下肺脊柱旁,清楚的看到一大团被造影剂强化显影的组织,仔细分辨脉络,可以看到是左下肺动脉分支与其中一支回到心房肺静脉的直接连通了。

“这样的低氧,可以手术对吧?”沈医生同我核实一遍。

“慢性的低氧,不妨碍手术,你只管放心做。”我把上午的话再重复一遍。

“手术后需要转给你做治疗吗?”他继续啰嗦。

“不用的”我简单利落地挂了电话,把专业医学网上关于肺动静脉畸形的专题内容转给他,请他自己去了解。他会看的,我们医院的医生已经习惯了这种学习方式。

邱阿姨已经77岁,耐受慢性低氧状态,这是她生活的一部分,也没有不良后果,一般不需要选择手术治疗,但需要告诉患者定期复查CT以及有症状及时就诊。

自此,“很奇怪”的邱阿姨的诊断告一段落。

我把她非常典型的CT图片存在手机里,准备在带教的时候给住院医师们讲述这个故事,-—我可能是有点职业病了,这是一个很有意思的教学案例。

这不算是一个常见病,在临床思维培训中,当遇见少见疾病、临床信息有不足、后续治疗效果不能证实初诊的判断,或者医生经验不足的条件下,一般倾向于采用“逻辑分析与假设推理“的方法来诊断

回溯到疾病的病理生理学机制进行思考就是其中一种。作为临床教师,通过让住院医师对案例信息进行分解,还原诊断与鉴别诊断思路的过程,有助于他们学习与养成规范的临床思维。

我最后弄清了患者诊断,这中间的经验可以传递给住院医生。

也得感谢我的那位“兄弟”沈医生,值得表扬的是他保持"很奇怪"的直觉与非要搞清楚不可的执着劲。

对常规不能解释的医学现象保持警觉,并知道如何调动资源、寻求帮助也是成熟的医生的一项能力。往大了说:这是现代医学的科学精神展现。适时打打"兄弟牌"也是非常必要的。
