研究认为，血清总同型半胱氨酸降低可减少中国成年高血压人群的首次中风风险，血清总同型半胱氨酸水平有望作为叶酸卒中预防的有效指标

近日，研究人员在针对中国脑卒中一级预防试验(CSPPT)的事后分析中考察了血清总同型半胱氨酸（tHcy）水平变化与中风之间的关系。

研究人员考察了16867名患者基线以及研究结束时tHcy变化。研究的主要终点是首次中风，次要终点是心血管复合终点事件包括心血管死亡、心肌梗死和中风。

研究平均随访4.5年，首次中风患者其tHcy水平下降的比例较少(β = -5.7； 95% CI，-8.8--2.6)，tHcy每降低20%，中风风险降低7% (HR， 0.93； 95% CI 0.90-0.97)。tHcy下降1/2-3/4的患者，相对于下降1/4的患者，其中风风险显著降低(HR 0.79； 95% CI 0.64-0.97)。对于不同年龄、性别、叶酸治疗、亚甲基四氢叶酸还原酶C677T基因、基线tHcy 、血液叶酸水平以及血压控制效果的患者，tHcy减少带来不同的中风风险效应。

研究认为，血清总同型半胱氨酸降低可减少中国成年高血压人群的首次中风风险，血清总同型半胱氨酸水平有望作为叶酸卒中预防的有效指标。
