慢性噪声暴露会增加心血管疾病(CVD)的风险;但是,混杂因素的作用和潜在机制仍未完全明确。杏仁核是参与压力感知的边缘中心,参与对噪声的生理反应。较高的杏仁核代谢活性(AmygA)通过参与动脉炎症(ArtI)增强的机制而增加了CVD风险。近日,心血管领域权威杂志Eur Heart J上发表了一篇研究文章,在这项回顾性研究中,研究人员评估了更大的噪音暴露是否与更高的:(i)AmygA、(ii)ArtI和(iii)重大心血管疾病不良事件(MACE)的风险有关。

没有CVD或活动性癌症的患者进行了临床18F-氟脱氧葡萄糖正电子发射断层显像/计算机断层显像检查,并测量了杏仁核的代谢活性和ArtI,以及判定了5年内的MACE。研究人员估计了每个人的家庭住址平均24小时的交通噪音和潜在的混杂因素。

在中位4.06年期间,40名参与者经历了MACE。较高的噪声暴露(每增加5 dBA)可预测MACE[风险比(95%置信区间,CI)为1.341(1.147-1.567),P<0.001),并且对多变量调整后仍然保持稳健。较高的噪声暴露与AmygA [标准β(95%CI)为0.112(0.051-0.174),P<0.001)和ArtI [0.045(0.001-0.090),P=0.047]相关。中介分析表明,较高的噪声暴露与MACE通过一系列机制相关,其中包括升高的AmygA和ArtI,占这种相关性的12–26%。
