Neurology:自发性脑出血,警惕早期死亡

目的:评估自发性脑出血(ICH)患者早期(48小时内)及48小时到30天之间病死率的时间趋势是否会有所不同。方法: 研究者应用基于人口登记记录,前瞻性收集1985和2011年间,在法国第戎地区的所有颅内出血患者(151000居民)。分析1985-1993年,1994-2002年及2003-2011年间,这三个时间段30天的病死率的时间趋势。校正人口统计,危险因素,严重程度和ICH位置后,应用Cox

结果:共记录531例ICH患者(平均年龄72.9± 15.8岁,女性占52.7%)。随着时间的推移,1985-1993年,1994-2002年及2003-2011年,30天内的死亡率分别为40.9%,33.5%及29.6%(1994-2002年,校正HR: 0.71,95% CI: 0.47-1.07,P = 0.106,2003-2011年,校正HR 0.49,95% CI 0.32-0.73,P<0.001)。在整个研究期间,脑出血发病48小时内,一个月内死亡率为43.6%。在最初的48小时内死亡率并未随时间变化而变化,但48小时到30天内的死亡率随时间而下降(与1985-1993年相比,1994-2002年的HR 0.53,95% CI 0.31-0.90,P = 0.02和2003年-2011年HR 0.32,95% CI 0.32-0.55,P<0.01)。

结论:在过去的27年中,30天病死率显著下降。早期的(48小时内)病死率没有改善,ICH急性期的管理仍需改善。
