中国神经调控的应用现状及发展

神经调控是近二十年来医学科学领域发展最为迅速的学科,其依托于医学生物及医学工程技术的发展,神经调控为很多疾病的治疗带来了颠覆性的改变,这一治疗过程主要作用于神经系统,但是也可以影响到人体全身的任何器官和组织,涉及众多学科领域,神经调控技术的发展对功能神经外科产生了深远而持久的影响。

神经调控是近二十年来医学科学领域发展最为迅速的学科,其依托于医学生物及医学工程技术的发展,神经调控为很多疾病的治疗带来了颠覆性的改变,这一治疗过程主要作用于神经系统,但是也可以影响到人体全身的任何器官和组织,涉及众多学科领域,神经调控技术的发展对功能神经外科产生了深远而持久的影响。

神经调控广义的定义为在神经科学层面,用电或化学的方式, 通过改变神经系统功能或状态而获得治疗效果的治疗模式。确切地讲, 是通过植入或非植入式设备, 利用电或化学手段, 通过影响神经系统中的信号传递, 兴奋、抑制或者调节神经元及神经网络活动等方式,最终产生治疗作用的方法。

神经调控包括有创神经调控和无创神经调控。有创神经调控包括电刺激、磁刺激、光刺激、声音刺激、药物泵等方法。以电刺激为例,即通过植入性电刺激装置,利用低水平慢性刺激对神经系统,包括脑、脊髓及外周神经产生作用,从而达到治疗目的;电刺激又分为脑和脊髓疗法、外周神经刺激、脑神经刺激、功能电刺激。刺激技术以微创、可逆、可调控的优势,在很大程度上已经逐步取代了毁损术, 如药物泵,即通过可植入性装置,采用神经系统内药物缓释技术,向神经系统注射药物以达到治疗目的,提供了针对靶向目标的精准治疗方式。无创神经调控包括耳迷走神经刺激、经颅磁刺激、超声刺激、光刺激等方法。

目前,脑深部电刺激的应用对脑功能神经疾病的治疗获得了新的进展。1954年,Poole 利用电刺激作为外科手段尝试治疗精神类疾病;20世纪70年代早期,Bec h t e r e v a利用植入电极使用“治疗性电刺激”治疗帕金森病( P D ) ; 2 0世纪70年代至80年代,植入式脑深部电刺激(DBS)系统开始被用于不同位点PD、非PD性震颤、肌张力障碍的治疗;1987 年,Benabid等运用DBS方法刺激患者丘脑长期治疗PD的震颤症状;1993—1994年,丘脑底核DBS、苍白球DBS开始应用于PD的治疗;1997年,食品药品监督管理局(FDA)通过了DBS作为特发性震 颤和PD震颤的治疗;2002—2003年,FDA 分别通过了DBS作为PD和肌张力障碍的治疗方法。

调控设备国产化是神经调控事业在我国快速发展的标志,2009年我国完成第一项DBS临床试验,至今已经完成1 万例次,市场占有率超过60%。国产的迷走神经刺激2014年进行临床试验,现在国内的临床占有率超过了70%,骶神经刺激已完成动物实验,进入临床试验阶段,脊髓电刺激亦将在近两年内进入临床试验阶段。

目前, 神经调控技术可以用于全身多个系统疾病的治疗,包括:癫痫、帕金森病、抑郁、阿尔茨海默病、特发性震颤、疼痛、泌尿系统疾病、心脏疾病、糖尿病、肥胖症等,随着医学科学技术的发展与进步,神经调控治疗将应用于更广泛的领域。
